Looks like scabies, can be gotten from use of public toilets or generally unclean toilets and/or dirty underwear, its basically insects living under the skin and laying eggs, itches like hell, theres a topical ointment you can use for it, just go to any pharmacy and buy it, shouldn't cost more than 300, the name has skipped my mind tho. You're welcome

I suspected same but to make assurance doubly sure, they should visit hospital to confirm exactly what it is ....before treatment can be administered

If you apply it on him for 5 days and no visible positive result please stop and take him to hospital to see a dermatologist.
