Yes. You'd fill another DS-160 using the details of the new school, update your cgi profile with the new DS-160 number and submit.

None of them.

Take your passport, signed I-20, DS-160 confirmation page, SEVIS fee payment confirmation, visa fee receipt, passport photographs in the right size, bank statement (if applicable), scholarship/funding letter (if applicable), admission letter etc. I think these are the most important ones. At your discretion, you can go with transcript, test scores, certificates and other documents you deem necessary.

you can re-apply for Emergency to Lagos. However, before you re-apply, ensure you can sufficiently answer "what has changed in your application" if not, i suggest you be patient with this whole thing

There are two dates on your I-20. Did you use the earliest date or the later date on the far left that says “start of class”?

What date exactly did you apply for the expedited date?
