1. The US is not meant to fight Russia directly because they are both nuclear powers. However, once Russia launches nuclear weapon on Ukraine soil to a devastating effects, all bets are off. US, with or without NATO will fight Russia with a conventional weapon. Some retired and active operatives were interviewed about what the US would do incase Russia fires nuclear weapon in Ukraine, it makes for an interesting read:

and know that Bush Senior ordered all the US tactical nuclear weapons to be removed and destroyed when he was the US president. Russian chose to keep their own. 2. US and NATO have the means to declare no fly zone in Ukraine. They dont just want to do it so as not make NATO to be an offensive unit. Note that I stressed something - US, with or without NATO, will fight Russia with CONVENTIONAL means if Russia attempts to launch nuclear weapon. And before US does that, it will ensure it raises all the dust in the UN to make country aware of the implication of Russia action, and by extension turning Russia to a Pariah state. Then, US will directly engage Russia with conventional means.

If Putin fires nuclear again, then US will be forced to do the same. And if Putin fires on the US, article 5 will be triggered and NATO will be drawn in. US will want to rally the rest of the world that Russia is an irresponsible country. And before Russia can fire an ICBM, it takes hours. It will have to be transported over along distance to a place where it will be fitted to a missile. US sees all these, and US has the means to attack the control of Russia and make the ICBM fails or a pre-emptive strike.
