Tubal disease affects approximately 25 per cent of infertile couples, and varies widely, ranging from mild adhesions to complete tubal blockage. Treatment for tubal disease is commonly surgery and, owing to improvement in microsurgery and laser technology, success rates (defined as the number of women who become pregnant within one year of surgery) are as high as 30 per cent with certain procedures having success rates up to 65 per cent. The development in IVF has now made it a suitable alternative to tubal surgery.
The main causes of tubal damage include:
Bacterial and viral infections
They are usually sexually transmitted infections, such as syphilis, chlamydia and gonorrhoea, which often result in scarring. A specific example is hydrosalpnix, a condition in which the fallopian tube is occluded at both ends and fluid collects in the tube.
A patient with hydrosalpnix going through IVF will experience difficulty conceiving because the fluid that is produced can damage the embryo.
Abdominal diseases
The most common are appendicitis and colitis (inflammation of the colon), high fever as in malaria. Abdominal diseases cause inflammation of the abdominal cavity, which can affect the fallopian tubes and lead to scarring and blockage.
Previous surgery
Previous surgical operations are also causes of tubal disease and damage. Pelvic or abdominal surgery, such as appendectomy for someone with appendicitis can result in adhesions that alter the tubes in such a way that those eggs cannot travel through them.
Ectopic pregnancy
An ectopic pregnancy is a potentially life-threatening condition that occurs in the tube itself. Even if carefully and successfully overcome, it may cause tubal damage
Congenital defects
In rare cases, women may be born with tubal abnormalities, usually associated with uterus irregularities.
Endometriosis
Approximately 10 per cent of infertile couples are affected by endometriosis. Endometriosis affects five million women in the US and six or seven per cent of all females. It has been linked to late marriage due to prior occupation with the busy schedule of work by these women, who delay having children till the late mid 30s or early 40s. In fact, 30-40 per cent of patients with endometriosis are infertile. This is two to three times the rate of infertility in the general population. For women with endometriosis, the monthly fecundity (chance of getting pregnant) diminishes by 12 to 36 per cent.
This condition is characterised by excessive growth of the lining of the uterus, called the endometrium. Growth occurs not only in the uterus but also elsewhere in the abdomen, such as in the fallopian tubes, ovaries and the pelvic peritoneum.
Symptoms often associated with endometriosis include heavy, painful and long menstrual periods, urinary urgency, rectal bleeding and premenstrual spotting. Sometimes, however, there are no symptoms at all, owing to the fact that there is no correlation between the extent of the disease and the severity of the symptoms.
The long term cumulative pregnancy rates are normal in patients with minimal endometriosis and normal anatomy. Current studies demonstrate that pregnancy rates are not improved by treating minimal endometriosis.
Abnormalities of the female reproductive organ
At least 10 per cent of all cases of female infertility are caused by an abnormal uterus. Conditions, such as fibroid, polyps, and adenomyosis(over enlargement of the endometrium), may lead to obstruction of the uterus and Fallopian tubes. Also, congenital abnormalities, such as septate uteru (a double cavity uterus), may lead to recurrent miscarriages or the inability to conceive.
Approximately three per cent of couples face infertility due to problems with the female cervical mucus. The mucus needs to be of a certain consistency and available in adequate amount for sperm to swim easily within it. The most common reason for abnormal cervical mucus is a hormone imbalance, namely too little estrogen or too much progesterone.
Behavioural factors
It is well-known that certain personal habits and lifestyle factors impact health. Many of these factors may limit a couple’s ability to conceive. Fortunately, many of these variables can be regulated to increase the chances of conception and one’s overall health.
Diet and exercise
Optimal reproductive functioning requires both proper diet and appropriate levels of exercise. Women who are significantly overweight or underweight may have difficulty becoming pregnant. We have been able to assist a significant number of patients to get pregnant ordinarily or following filed IVF cycle by getting them to go through a Mayr therapy and losing between 10 and 15kg in 10 days.
Drugs
Drugs, such as marijuana and anabolic steroids, may impact on sperm count in men. Cocaine use in pregnant women may cause severe retardation and kidney problems in the baby. It is, perhaps, the worst possible drug to abuse during pregnancy. Recreational drug use should be avoided when trying to conceive and during pregnancy.
Environmental and occupational factors
The ability to conceive may be affected by exposure to various toxins or chemicals in the workplace or the surrounding environment. Substances that can cause mutations, birth defects, abortions, infertility or sterility are called reproductive toxins.

The feat was recorded by The Bridge Clinic, Lagos, on February 16, with the delivery of a male child, named Tiwatope.
The oocyte was preserved through cryopreservation, which is the cooling of cells and tissues to sub-zero temperatures to stop biological activity and preserve the cells for future use.
Human oocyte cryopreservation (egg freezing) is a process in which a woman’s eggs (oocytes) are extracted, frozen and stored. Later, when she is ready to become pregnant, the eggs can be thawed, fertilised and transferred to the uterus as embryo.
According to the fertility physician with The Bridge Clinic, Dr. Emmanuel Owie, who broke the news, Tiwatope’s birth was significant in many respects, as he puts the country on the global map in the practice of oocyte cryopreservation, a new offering in the in-vitro fertilisation (IVF) space.
“Before his birth, this new practice seemed to be an exclusive preserve of the developed world of Europe and North America. It is even more significant considering the fact that since the report of the first pregnancy through this protocol in 1986, the practice has resulted in the birth of only about 5,000 babies worldwide,” he stated.
Dr. Owie said Tiwatope’s mother had her eggs frozen using the vitrification, also known as flash-freezing process – the cutting edge technology in cryobiology, where the eggs or oocytes of a woman are dehydrated and the water content replaced with “anti-freeze” solution (cryoprotectants) before freezing. This will prevent the formation of ice crystals, which could destroy the cell.
Tiwatope’s parents had battled infertility for eight years, but the clinic had the mother’s eggs frozen for two months.
Dr. Owie explained: “We fertilised the eggs using a standard technique, known as intracytoplasmic sperm injection (ICSI) to overcome the egg shell, which normally gets hardened with freezing.
“The fertilised egg was subsequently transferred into her womb, resulting in the pregnancy of Tiwatope. She had her antenatal care in her family hospital and delivered the baby boy through caesarian section (SC).”
The clinic’s coordinator, Corporate Marketing/Communication and Client Relations, Dr. Ekundayo Omogbehin, said the baby and the mother were in good health.

Hi @Saronna. You should advise your sister that yes it’s tough to give up caffeine during pregnancy, but she should at least limit herself for the health of her baby. Caffeine can cross the placenta to your baby, who cannot fully metabolize the compound, and interfere with her sleep patterns. No studies have been done on humans to determine if caffeine causes birth defects but moderate levels of caffeine are deemed safe for pregnant women. I was diagnosed with endometriosis and doctor in Bridge clinic, advised me to take microgynon and Fertilovit endo, which ave been taking though the cost is high. I am just confused on maybe to try another clinic or maybe finish up the IVF process with them. I don't really have that satisfaction with the clinic. I have been ttc for close to 2 year. IVF is is the process of fertilization by manually combining an egg and sperm in a laboratory dish, and then transferring the embryo to the uterus. Hi my dear I would advise that delayed menstruation is a common problem with several potential causes. Stress can be an enormous contributing factor, but besides this, inadequate nutrition or excessive exercising could be the one to blame. It is common for women to use a lemon juice or lime juice solution with the belief that it prevents pregnancy or sexually transmitted diseases. Undiluted lime juice does slightly alter the cycle causing a delayed menstrual cycle. I would say consult a fertility consultant to run tests and to assist you on the next steps. Otherwise, all will be okay. Keeping you in my thoughts and prayers. But since then the slight blood that i have been seeing stopped. Throughout the period i breastfeed my child no slight blood or menstruation again . I have stop breastfeeding for 5months now & my child is now 1year&8month, and i have not seen my period. I went for medical care, i was told its infection, i treated it & it was confirm that i don't have infection again, i was given a pill to take for 30days so that i can menstruate, i have finished it but yet no menstruation.Please what can i do?That is my first child. I need experience mothers & medically experience people to advice me. Could it be the prevention i was using that stop my menstruation or it is normal?. |

My period was to show on or before the 17th of this month.I was feeling somehow though ( to be candid)this morning I woke up to do a pregnancy test,thanks God the conceived plus lubricant that we bought came with loads of ovulation test kit,and pregnancy test kits.lo and behold it came out with two lines,and today my four years wait ,cries,anguish, prays,has been answered by the Almighty Jehova the creator of heaven n earth,the Alpha and the Omega.pls join me to thank God.I feared HSG so much,and my creator didn't allow me pass thru it. |

I am Precious, a retired nurse and I will say that, brown discharge could be a sign of miscarriage or may even result from a condition that is unrelated to the pregnancy. It is advisable to seek medical attention for any brown spotting that doesn't fit the criteria of implantation bleeding however no two cases are ever the same. I will keep you in my thoughts and prayers and I hope all goes well.
