Please get a lab test, blood test, BUN, CBC, Creatinine , troponin liver enzymes test, GI test, gall bladder test , and other test that can rule out if the Liver , heart , kidney is healthy or not . Ask if he still make urine and go to respectable hospital in Nigeria , see a respectable doctor who is knowledgeable, not the quack one. Lab test and adequate diagnostic test can eliminate guess work and can pinpoint what is going with your brother. It could be acute Heart disease or Acute kidney disease or Cushing syndrome , I do not know. Limit salt intake , fluid restriction and go see a doctor ASAP

What do u mean by acute when a symptom has lasted for up to 1 yr?
I've never heard of acute heart disease throughout my medical school and acute kidney disease doesn't present like this

He doesn't need all those investigations if u know what u are doing

Looks like hypothyroidism. Neck swelling may be due to goitre. Generalised body swelling secondary to myxoedema.

His sleeping a lot also can be a symptom of hypothyroidism. Breathing difficulties could be because of the mass compressing his trachea or effect of hypothyroidism too.

Since it's refractory to treatment which form the pictures are essentially a cocktail for reducing edema secondary to reduced oncotic pressure, I am leaning heavily on another underlying cause.

Is he constipated all the time, cold to touch, hoarse voice?

I believe the summary of the above is to take him to a good federal tertiary hospital with many specialists that can put their heads together to come up with something and fast...

Kidney disease may be asymptomatic until when eGFR is less than 30mls/min/1.73m square body surface area then he starts presenting with uraemic symptoms

They might be treating him for kidney and the illness may not be kidney disease. Knowing nigeria's constant medical misdiagnoses. It clould be alergy.

It may not be an allergy.. this has been on for more than three months... He should post the results of S/E/U/CR as well as urinalysis results
