Start with poultice of Castor plant. Put it round all areas that is swelling.

Guy, my son of 5 years had similar symptoms. We have done a lot of investigations and all prove abortive cos Doctors couldn't phantom a name attached to it. We were only told that my son protein is low that we should feed him more of protein foods. At some point his Penistone and scrotum started swelling. Yes he can be able to urinate but the kidneys are not functioning at optimum level to remove toxins from the body

This is definitely Nephrotic syndrome. He needs to visit a teaching hospital or any other specialist hospital. If treatment is initated early enough, he will go into remission within a month.

They might be treating him for kidney and the illness may not be kidney disease. Knowing nigeria's constant medical misdiagnoses. It clould be alergy.

It may not be an allergy.. this has been on for more than three months... He should post the results of S/E/U/CR as well as urinalysis results

I don't have strength going through comments to check if solution had been provided. I experienced this when I was in my secondary school day. It is called Hay fever. I was frequent bathing @ dirty river and doing fishing and catching crabs for fun. I was bitten by something in the Riverside. In fact the thing affected both me and my brother.

The swelling moves from one location to another haphazardly.

Solution: A lot of antibiotics will cure it gradually and not hastily as you want it. Beecham Ampiclox will do. Talk to your doc about Hay fever.

His sleeping a lot also can be a symptom of hypothyroidism. Breathing difficulties can be secondary to the mass effect of the goitre compressing his trachea.

Since it's refractory to treatment which from the pictures are essentially a cocktail for reducing edema secondary to reduced oncotic pressure, I am leaning heavily on another underlying cause.

It's really unwise to come to a public forum to seek medical advice for several reasons - one being that most people on here are basically unqualified to offer medical advice.
So, you're basically setting up a recipe for anxiety.

There are several differentials in your brother's case, some potentially grave, others not so much.

Paroxysmal Periorbital and airway edema in the absence of other systemic symptoms may be suggestive of Hereditary Angioedema, and if so, the Lisinopril tablet would just worsen the condition.

Anasarca (whole body edema) could suggest liver or kidney disease, as heart disease most commonly causes dependent edema (from the legs upward). In the presence of lethargy (sleeping a lot) and absence of signs of liver failure (like Jaundice), I'll lean towards a kidney disorder - nephrotic syndrome.

Bear in mind that Nephrotic syndrome is not a diagnosis, there are several etiologies, some relatively benign, others potentially catastrophic - a nephrologist would know best how to diagnose the existing disorder.

However, I'll reiterate that it is extremely difficult to make a diagnosis over a public forum without proper physical examination and history (history is so important because a single story from you could totally change the diagnostic workup).

Contrary to what most people here are asking you to do, I'll advice you guys visit a good general practitioner first, who'll examine then refer you guys to the appropriate specialist.
Primary care is important.

with respiratory embarrassement esp with accumulation of fluid in the chest.
