Hi. Sorry to have been AWOL on this thread - I was bereaved.

We unfortunately lost the top person in the health team where I practice, after a brief illness. This put a lot of my activities in disarray for a while.

Death. The ultimate inevitability. Physical death, that is. I believe in the lifeafter. . .

Interesting topic.

I have a patient on the ward right now who's billed for an Elective Caesarean Section tomorrow - the indication: she's had 3 previous Caesarean Sections.

So, this shall be the fourth. And it shall be the last, also, because she has consented (I'm not sure her husband was in agreement) to have both her tubes tied during the procedure. Technically, therefore, the tomorrow's procedure is an Elective Caesarean Section with Bilateral Tubal Ligation.

Wish us good-luck and God's speed, folks.

hi doc pls i need help. i have this problem of oversweating on the palm, feet and face. also i tend to get too nervous easily over little things and this also ends up in me sweating over the face. pls what can i do?
Generally speaking, people have varying degrees to which they produce sweat and nothing can change this.

Sweating in the palms when one is nervous, is an autonomic nervous system response when one is tense - nothing much you can do to change this automatic body response. Your best bet therefore, is to adapt ones self to those tense situations such that you learn to feel less nervous under such circumstances. The sweating should then lessen.

What have your doctors been saying is the diagnosis, I wonder.

Invest time and money to see the right specialist, this time - the Gynaecologist. Oh, hang -on! TTC is another threads acronym for Trying To Conceive. Which suggests you are probably seeing a Gynaecologist - please stick with them and I'm optimistic you will conceive pretty soon; don't give up.

hi doctor.thanks for the reply bt i 4got to mention dt they did a urine test  and the result said it wz a staph the doc gave me antibiotics but like i said ealier i still ave the discharge with odour and i itch too.pls what do i do.
Any competent doctor knows that Staphylococcus is nearly always a normal commensal in and on the body and hence should be ignored; almost to the point of seeing it as a contaminant of specimen culture results. This is what I referred to earlier in my reply comments about Staph, above.

You should then go ahead with my suggested prescription, above.

Dear Doc,

Sometimes on the 12th of September i have a protected sex and i saw my period on the 25th of September although i fell sick and i did test i was told i had typoid/malaria parasite which was treated half way because i stop using the drugs cause it was having reaction on me. i was suppose to see my menses 22nd of october following the 28day stuff but today being 25th am yet to see it.

What then is the problem.
You may quite simply be pregnant. Always see a doctor rather than going to do a test by yourself.

I'll explain further. It is possible that the flow that started on the 25th September, was not a menstrual flow at all. It could have been the implantation bleeding that often times occurs in early conception - and it does so usually around the time of the expected period, hence the more easily confusing for women. It however is usually not as heavy and as prolonged as the normal menses.

In other words, you may have been in very early conception and the accompanying "morning sickness" is what your test was calling typhoid/"malaria parasite".

See your doctor, pronto.

I think she may or may not be pregnant.

I'm no Gynaecologist but it is a fact that depending on how often one takes hormonal contraceptives such as Postinor in a given time period, this may cause disruption of the menstrual cycle thus possibly causing delay.

Let her get it sorted out with a visit to her doctor.

Your best hope still lies in seeing a competent general duty doctor whenever you get ill. There is no more I can do by way of assistance. Well, may be there is a final further step: see the specialist (will cost more effort, time and money, though) - and in this case the specialist to get a referral to is the Consultant Respiratory Physician. You'll get them in any General or Teaching Hospital by way of a referral letter from your doctor.

Psoriasis should be managed by a Consultant Dermatologist - they are the ones that would be up to speed on the right drug combination. Left to me, I would be at a loss, after writing Prednisolone, for what further to include on the prescription list.

Hello. Based on the scan findings of a Pelvic Inflamatory Disease (PID), and not any suggestion of "staph" per se, I'm going to give you a good-quality, broad-spectrum antibiotics prescription many times, the poor quality of the drugs is the problem:

1) Tablet AUGUMENTIN 1 gram (1000mg) twice daily for 1 week - get the Smithklime Beecham brand.
2) Tabet METRONIDAZOLE (e.g. Flagyl) 400mg three times daily for 1 week - any regular brand should do.
3) Intramuscular Injection GENTAMYCIN 280mg daily for 1 week - any available brand with 280mg per vial, will do.

Who would you get to administer the said intramuscular injection, I wonder?

What readily comes to mind as a possible cause, is exposure of your palms to repeated abrassive force, as in the case of an auto mechanic who works all day without protective gloves.

Rare skin conditions are possible causes. Growth hormone abnormalities, possibly.

A doctor will determine if the condition is a disease that requires treatment or just a reaction to the kinds of exposure mentioned formerly - in which case, prolonged abstinense from such harsh exposure to the palms, will gradually revert the texture back to a more subtle one.

Doki Balogun,
How can I be free from asthma. I do have difficulty in breathing many times at night and some times I eat beans. Sometimes when I drink beer with just 2 stick of ciga.
I have battling with it for over 9yrs now. There was time I stop smoking, but now I've started it again.
Cold, dust/fumes and physical/emotional stress. These are the main factors that trigger-off an Asthmatic attack. Obviously, the cigarette smoke falls under "fumes" as a possible trigger in your case.

The good news is that Asthma most times wears-off as one advances towards middle-age and beyond, assuming of course that the condition began manifesting in childhood or the teenages, as do the majority of cases.  It less commonly begins to manifest for the first time in adulthood.

What's your age, bro?

Please, what is the cause and what do you advice cos it gets really uncomfortable at times.

I'm afraid I cannot decide what the problem can be with only one variable to work with - the symptom of heat that you mentioned.

Whereas, if I were to see you one-on-one, I'd ask a lot of questions then do a clinical examination of several systems of your body. I'd then have much more data to allow me reach a conclusion.

@the doctors in the house.

What is the cause of my unusual ache when I go jogging? Over the summer, I was an active daily jogger, then I stopped. Nowadays, am trying to pick it back up with much difficulty. Everytime I go jogging, a mild ache starts up from the right lateral side of my navel, and eventually moves up to the my ribs. Sometimes the pain is so severe that I have to stumble home.
And sometimes my ankle gets painful, that it becomes hard to jog.

What could be the cause? Lack of proper nutrition, is my body getting used to exercising again, or some other internal problem?

thanks. embarassed
I'm guessing that you are simply lacking your old stamina. Heavy breathing causes pain under the diaphragm, and this seems to be what you described. Pains in your ankles can be explaned by the srain from the jogging.

However, to be sure, check with your doctor to rule-out a possible heart condition - I'm taking caution of the sometimes "so severe" chest pains you mentioned.

Your need for more information, added to all the data you seem to have already, by now any doctor would be concerned as to whether you are seeing a doctor one-on-one to sort through all of the information.

Half-knowledge is dangerous. Especially in a zero-regulated health environment such as ours, where one can pick-up Clomid or any drug for that matter, just like that; without a doctor's prescription. Dangerous indeed.

Let the specialist do the job.

Hello ladies,

I'm afraid I've committed lookery. Might as well go the whole hog and then commit postery  grin

The menstruation is said to be regular, when it comes at regular intervals
. The average is 28 days interval. But there is an accepted normal range: 22 - 36 days.

So, if a lady usually sees her menses every 24 days, as an example, this is normal. And as long as it comes roughly every 24 days interval, it is then said to be regular for that particular lady since that is her usual interval. The regualrity therefore, refers to the rhythm: every so-and-so days interval - and it sticks to that interval if it is regular.

Imagine an event occuring every 6 days, starting from Monday. The next event therefor would be the Sunday following. Then Saturday, and so on. The day of the week changes, but the event still stays regular at every 6-days interval.

The same thing with a regular period. The dates may change from month to month, but it is the interval that determines regularity. Except, of course a mentrual period that comes every 30 days - the date then tends to stay nearly the same, month after month, since this 30-day interval resonates well with the 30-days that makes up a calender month, more or less.

Forgive my intrusion, ladies.

Thx Surgeon,  Yes by Vitabiotics. For very long time i av been taking Ventolin and predisolone tablet due to challenge on my health and this makes me to loose weight and affects my menses i,e not flowing regularly, it comes on interval of 2 or 3 months.  Though im advance in age and not marry, i tried Phamaton capsules but not really get the result i wanted.  I surf on internat and discovered that an ailment can cause irregularities in woman circle. In a conversation a lady said that she takes WELL WOMAN and good for her skin, menses etc.  Pls is there anything i can use to make my menses flow well.  Thanks
Some of our women folk wrongly believe that it would result in a problem, if their menses doesn't flow well - meaning that the waste-products that's supposed to flow out, still remains in their system. Well, like I said, this is absolutely wrong thinking. The menses is no waste product at all. It is the endometrial lining shedding-off due to the newer one growing beneath. So, if there is no new growth beneath, their would be no new flow - doesn't mean danger or any retained waste, etc.

I'm saying this because I have noted you mentioned being advanced in age and unmarried, hence I assume you aren't trying to get pregnant - this would have been the only reason I would have been concerned about your irregular menstrual cycle.

If you still are concerned about this aspect of your health, I would suggest you see a Gynaecologist

teetee2:
Is it a must for women to take WELL WOMAN and then what is the functions it renders to the body. Thanks
I assume you refer to the Vitamin suppliment from VitabioticsI did a quick search. It is just that - it adds vitamins and other mineral substances needed for a balanced diet.

It isn't a must. It all depends on ones state of health and dietary needs. If one is eating a fairly balanced diet and is in general good health, vitamin suppliments are not a MUST. The extra that the body will not absorb would be passed-out in the stools.

An active sports woman on the other hand, would need more vitamins because she has a higher turn-over of these minerals due to a higher metabolic activity during regular training and competition, etc. Same thing applies to a pregnant woman - her Gynaecologist would recommend some mineral suppliment or the other.

hi surgeon
thanks for your suppport
as a final request please could you suggest specific food items rich in fibre as you suggested.
The green stems of plants (vegetables), their leaves and their fruits - humans cannot digest the cellulose contained in the cell wall of these plant structures. As such, they remain as bulk in the stools.

So, in general, we say fruits and vegetables are rich in fibre. Even the squeezed-out juice from Citrus fruits such as Pineapples or Oranges - they still contain lots of tiny pulps from the fruit-flesh, hence are rich in fibre also.

thank you. but how come there is still blood wheni have a running stomach and  the stool is watery. hence i dont think its due to friction or difficulty in movement.

please  help
Sometimes when stools are little and watery, they become a nuisance in terms of the method the intestines adopts to empyt them. The intestines are convoluted loops of many meters length an so you can imagine fluid faeces hiding here and there in the various troughs within the loops.

The intestines function better with bulky stools. With loose stools they work inefficiently with the individual having to strain automatically as an aid. This is why the anus soon becomes oedematous(engorged) from all the straining, which may then lead to engorged veins (Haemorrhoids) that may then bleed. Therefore, one can develop Haemorrhoids both from Constipated stools and Diarrhoeal stools - the common factor is straining.

This is all in a general sense. In some cases of Diarrhoeal Disease, watery stools run out very easily with no difficulty whatsoever.

Please what do i do next? I'm so worried!

Hi Doctor

I observe that when i consume sugary products, there is blood in my stool. So i stopped, but anytime i take coke or anything like that it starts again. Please what is the cause for this

Thank you
The only way Sugar is connected to a change in stools in some individuals, is that it changes the consistency of the stools - makes them soft, sticky and generally more difficult to move along the bowels, hence causes the defaecation to be more difficult; usually with straining and invariably some bleeding at times. Unlike Fibre-diet such as diets rich in Fruits and Vegetables - these cause one to stool with ease and efficiency because they add bulk to the stool and bulk greatly aids the peristaltic (pumping) action of intestinal motility.

If I am correct, you would notice a change in the manner and consistency of your stool. Otherwise, it is not Sugar causing the bleeding, but something else.

Add more Fibre to your Carbohydrate/sugary diet. This should help correct the problem.

A`Heart Attack would give rise to a chest pain usually as a result of increased Heart activity during physical exersion (like climbing several flights of stairs) or emotional aggitation/excitement.

In your case, you describe feeling chest pain only when the chest wall undergoes expansion (breathing-in) or contraction (bending forward; lying on the side, etc). It points more to a problem of the connective tissue - i.e. of muscles, tendons or ligament, or possibly a Lung disease.
