Year in, Year out, the huge gap between the perfection of the London Bus service versus the Lagos transport system keep rising.

I'm sure the long-term goal in Lagos is to eradicate severe go-slow, and create more time-saving means of intra-city transport.

The present problems, like you already know, include; Excessive amount of private cars on the roads, Very few BRT buses, and the badly-mannered Yellow Danfo buses.

The present successes are: Widespread eradication of Okada riders in major areas, and Provision of new 5000 buses in Lagos.

Short-term Solutions to Perfect the Transport systems are:
1. Create policies that charge car owners a yearly tax to pay for each car they drive. This fund can be used to Perfect the state òf the major roads. This will definitely discourage Lagosians from owning more than one car. The Rich will pay more money to drive more cars. This will definitely reduce Road traffic by almost half. The middle and lower class will be encouraged to take the air-conditioned buses at a discounted price, which is what we do in London.

2. Ensure the BRT buses go to more destinations within Lagos. This can be perfected by introducing mobile applications to track their buses, the scheduled time for each journey, and update them on any changes to their journey. This is what we use in London via Google maps.

3. Introduce the MOT system whereby the physical state of each car has to be tested yearly by the Transportation Engineers. Several cars in Lagos produce huge amount of smoke and noise which destroys our ecosystem. We need to ensure that only healthy cars are capable enough to enter our roads. This will make Lagos a beautiful place.

Long-term solutions are:
1. Professionally train the Danfo drivers on how to properly drive BRT buses, so that the government can employ these persons to professionally drive the BRTs.

2. Create intra-city trains. Unlike cars, Trains don't experience traffics beyond a few minutes. The London Underground trains have reduced road traffic by a very large extent. With trains, you can go from Lekki to Okoko within 30 mins. Lets stop procrastinating, this is 2018 already.

Please spread this vision. I hope it eventually gets to Governor Ambode.

I don't usually comment on NL, but I just couldn't withstand the avalanche of ignorance amongst the entire posts concerning this topic.

Simply the dude has a sexual disorder (a paraphilia) which has been (spiritually) remixed with a psychiatric condition called Trichotillomania (a condition whereby the affected individual enjoys eating human hair).

It's really a bad case medically. But a treatment is possible via the Cognitive Behavioral Therapy offered at Psychiatric facilities.

My advise to the prey: Please run for your life. He's not too far from being psychotic. Inform his relatives to get him down to the required facility as soon as possible.

It's Just another case of Vaginismus (also called Penis Captivus). About 5-10 cases are reported yearly in our continent. It's simply caused by Involuntary Contraction of the vaginal wall muscles (majorly the Pubococcygeeus muscle) against an erected penis, impeding relaxation and drainage of blood off the penis, mostly as a result of Anxiety, resistance or first-time sex on the side of the woman.

Treatment is simple: Both partners should calm down, and give the vaginal muscles some time to relax. Usually about 30mins-2hrs is required for best results. Do not panic. It should resolve in that time frame. If not, call an Ambulance (not the neighbours o). We mostly give a drug called Diazepam to assist the woman vaginal wall muscles relax.

In a country where things work, the first witness should have dialled 911 (Ambulance service). But here in Nigeria, a lot of medically-naive individuals exist. Yes I'm aware Ebola once existed in our land, but that doesn't mean you should generalize that all bleeding patients have the viral infection.

Just pad your hands with clothes, tilt her head to one side, and carry her immediately into a vehicle heading to a nearby hospital (or at least a clinic).

She probably have Disseminated Intravascular Coagulopathy (DIC), or an unidentified bleeding disorder, ruling out causes of Upper GastroIntestinal bleeding.

Treatment is simple: Transfuse Whole blood, or better-still, Fresh Frozen Plasma, keeping in-view any bleeding vessel, perforated viscus, or focus of infection. Investigation results should be pending.
She should recover within days.

Lisa, a good-looking tall and beautiful young lady, presented to the accident and emergency unit on account of abdominal pain and bloody vomitus of few hours duration, which worsened on feeding and didn’t resolve even at rest. She felt that was the end of her existence, but to her greatest surprise she got well-treated, counselled and returned back home relieved and enlightened.

In few minutes, we’ll be breaking misconceptions and wrong beliefs concerning the dynamics of this disease.

-What exactly is Peptic Ulcer Disease?

It is a disease condition characterized by presence of an ulcer in the surface layer of the lower oesophagus, stomach or within the 1st part of the small intestine (duodenum). It is common worldwide among persons aged 18-50 years, and occurs more in males than females at a ratio of 2:1.

-What is the cause of the condition?

Common misconceptions exist that prolonged hunger creates room for hungry worms to feed on the surface lining of the stomach, causing ulcer pain, and intake of food divert the worms to feed on the newly ingested food. The actual cause of PUD is a tiny bacterium called Helicobacter pylori. It is transmitted via ingestion of contaminated food. This microorganism creates a suitable alkaline environment for itself to survive within the acidic stomach. It then expresses genes and stimulate cytokines that alters the formation of the protective mucus-bicarbonate layer, giving room for damage to the stomach surface by the highly acidic gastric Hydrochloric acid.

-What are the risk factors of PUD?

Smoking, long term use of certain pain killers such as ibuprofen and other NSAIDs, and prolonged fasting in persons with previous history of this condition, are by far the top leading risk factors. Habitual use of coffee, stress and spicy food has also been implicated.

-How do affected persons present?

Classically, most persons develop upper abdominal pain, centrally localised, and commonly described either as a burning, biting or stabbing pain, lasting for more than an hour and sometimes disturb sleep. In ulcers affecting the stomach, the pain is worsened on eating, while in ulcers affecting the duodenum, the pain is relieved on eating. Passage of blood in stool or vomiting of blood indicates complicated states that warrant urgent hospital intervention. Death can occur due to severe bleeding or infections if treatment is delayed for any reason.


- Can this disease be prevented or even treated?

Definitely, prevention begins by enlightening your friends and family about the dynamics of the condition. Maintain a good feeding pattern, regularly wash your hands with soap and water, quit smoking and alcohol intake, and avoid use of NSAIDs (ONLY on doctor’s prescription). In-hospital investigations such as endoscopy, H.pylori tests and others must be carried out. In uncomplicated cases, use of Triple Therapy is gold standard. It involves the combination of 1 proton-pump inhibitor e.g. Omeprazole, and 2 antibiotics such as Amoxicillin and Clarithromycin. Antacids such as Gestid provide only temporary relief. Complicated states such as bleeding would require in-hospital blood transfusion, while perforation (formation of a hole) would require surgery for correction.
