Since she v had her baby, feel free to input every date.
You still have to book appointment with the US notary public. You can as well choose the same date ur interview is scheduled for if it's available. If you get your visa and will be traveling soon, there may not be any need for the form, unless you want her to get every doc ready before you arrive.

Take it to the embassy if your wife Hv had ur baby, you Hv to book appointment for this. It's free
or you take it to any notary public if she v not had her baby.( You and d notary should not put date ye[/font]t after signin) DHL to her datsall

My own thinking is for her to travel and walk in to any hospital visit the hospital instead of feeling any form before leaving naija.
