If your cousin actually selected "yes" to indicate a past denial, but the officer claimed he selected "no," there might have been an error in submission or miscommunication. He should retrieve his previous DS-160 confirmation page to verify what was submitted. Probably he couldn't demonstrate strong home ties and weak documentation. Let him work on those.

Yes, your passport number will change with anything that you do in your passport be it change of name/address/spouse name/passport renewal, etc.
