APPLICATION LETTER WITH REQUIRED DOCUMENTS a.Covering letter with address and telephone numbers b.Complete Form 1C c.Certified True copy(CTC) of title documents d.Current tax clearance of the assignor and assignee e.Four copies of the deed of which consent is sought f.Copy of approved survey plans g.Evidence of payments h.Where the property is covered by state leasehold of certificate of occupancy, evidence of ground rent up to date. i.A photograph of the property. j.A copy of the applicant and/or his agent legal representative's identification. NOTE: If required documents are not complete, a letter will be written to the applicant stating the missing documents. 2. FILE NUMBER IS ISSUED 3.
