I remember the first time I got a query. It really made me sad. To me, a query is almost like a sack letter. Till today, I still don't understand everything about the so-called 'love letter'.

Please I need someone preferably in a management position to tell us what we need to know about it. For example, is it true that you'll get fired after collecting three queries? Does all queries go into your file? Must you always admit you're at fault or can you defend your action/actions and not say sorry?
