i sit for approximately 12hrs a day. it has been going on for like 3yrs now

It's caused by over sitting, you need to start exercising,even for 30mins a day,you 'll see change, but try not to be sitting for that long.

No worries mate.I guess you do have some heart burn sometimes and you do eat late at night sometimes?

If this is the case, try not eating after 7 or 8pm latest.
