Defenders Branislav Ivanovic, John Terry and Gary Cahill are joined by midfielders Nemanja Matic and Eden Hazard plus striker Diego Costa. Southampton full-back Ryan Bertrand and Spurs striker Kane are also in a team voted for by top-flight players.
