I still don't understand all you people that are justifying cowardicial indecision with orders from your superiors.

What is right is right.

I am a Civil Engineer, and I have several times gone against the orders of my superiors when designing or Constructing in order to arrive at the safest and best solution on projects.

The worst thing they can do is sack you.

But if you fail to do the right thing, and a disaster happens....you cannot say you are not responsible, because you could have made a difference.

I have several Doctor friends, and these guys are totally dedicated to their patients. They will be ashamed to identify the OP as a colleague.
Imagine when I a novice in the medical would be looking helplessly at a patient, then a trained doctor would do the same.

You are wicked, how can order from above stop you from saving someone's life, if its your sister will you watch her die like that?
You are a doctor and thats your job irrespective of the order above.
My 'sister', Nigerian doctors don't feel their actions and inactions amount to wickedness or sadism! What do you expect from people that were trained under harsh and dehumanizing environment? The answer is indifference, nonchalance, sadism, mediocrity,et cetera! Nigerian doctors are just like Nigerian police officers!

I remember when you started to chastise the op. But I couldn't know when you changed. Since this is your annoyance, I will side with you that not all doctors are selfish. There are so many of them with the heart of Jesus. Similarly, the op is not wicked but inexperienced.

My dear u're wrong.
1. D guy is a House Officer, d lowest rung of d ladder. There's a limit 2 what he can.
2. Getting may not b d worst dat can happen. He cud lose his practicing licence if he makes grave mistake, esp in this era of medicolegal jamboree & dat means he wudn't b able 2 practice again. So, wot ll then b d consolation dat he did d right Thing?

Medical practice is a calling. Some go into it cos of "gud" pay (wc is not really wot obtains) while others cos of d joy & sense of fulfilment in transforming somebody's pain/tears into joy & lafta. Buh as u go along u learn 2 b Empathic buh NOT Oversympathetic, esp if u witness a case or more where a patient u battle 2 save his or her life comes against u in d court of law over a flimsy reason just 2 make cheap money.

Please, all the doctors in the house, if such emergency like the op narrated occurs again in Nigeria, please pay some money for the patient and start treatment, hopefully life would be saved. Then, come to the forum later and present the patient's case. You won't believe what could be realised for such patient.

Tears run down my cheek as I type this. I went into d medical profession to Save lives, but today I watched a patient die and I could do nothing.
It was around 11am today, 26th Dec 2015, I wz in the emergency room as the only Dr & House officer on call in a state Specialist hospital wen a peugeot wagon followed by a police van drove in. They opened d doors & trunk and started bringing out Accident victims. I did a quick triage & saw dat d 2 in d trunk were in a more critical condition, So d immediate plan was to refer d 2 to FMC wia dey hav better hands and equipment to handle it. So I asked d policeman who d driver is so dat d 2 will be rushed to FMC & he said he died at d Accident scene. I ran outside to buy airtime to start calling other HOs, b4 I could return, dey had dropped d 2 & drove off.
I called other HOs, only 2 showed up, others had travelled. To my greatest suprise, I called a medical officer who is a snr colleague in d proffession, she said I shud nt touch anyone till dey paid. That I shud Infact refer all of dem. I was incapacitated, d patients were in pains, crying, calling on us to help, some already unconscious, but we couldnt help cos we have an order frm above.
Within 5mins, 1 young lady in her early 20s died. Abt 10mins later, another police van came and carried d remaining 7 to FMC.
This evening, some road safety corps came to inquire if any Accident victims were brought to our center. They had retrieved d manifest frm d vehicle and contacted d families of d passengers. Dey came wit d relations of one of d patients, and wen I told dem 1 of dem died, dey asked for d Description of d 1 dat died and I showed dem d pix of d gal dat died, behold she was d 1 dey were looking for and right there was the gal's fiancee. Their traditional wedding was to be in 3days tym
I want to use this medium to call on the govt to make policies dat"ll enable Drs treat patients in such condition. Let's Save the human race

I have made this plea in one of my Facebook posts.Safe life first and bill latter in an emergency situation.

If there is a hell, I think the hottest part of hell is reserved for medical professionals who go against their oath to save lives, for the lure of filthy lucre. @OP: If you feel that you are not allowed to uphold your Hippocratic oath where you work, you should resign. Doctors are in high demand, you studied medicine to save lives.

As sad as it is, the OP is simply a reflection of medical practice in Nigeria. This is the same country where Doctors collect money from medical reps to prescribe their products, even if the products prescribed are at the detriment of patients.

Why would patients have to pay before they receive treatment at A and E? Shocking.
The reasons why Nigerian doctors are like Nigerian police officers are here!

@Op, you are very stupid and naïve.
How I wish the late lady's family will sue your sorry ass right now.

Something of dis nature happened, instead of complaining to your superiors, the hospital mgt etc, you came to social media to disgrace yourself, and denigrate your profession cos of your stupidity and incompetence.

Following orders from above why did you not immediately ensure that the patient leave that facility ASAP.

Do you think that with this level of ignorance you just exhibited here you can save that life even in the best of centres.

Which kind of medical school did you even attend??

Lesson: Always bridle your tongue.
Stay away from social media.
Improve Your skills and competence level.
Learn how to be a professional and always differentiate bw professionalism and sensationalism.

You are right, Bro. But, my question is : were his actions towards a critically ill patient justifiable, as a trained medical doctor that he is?

In a sane society the answer is no but in a country like Nigeria the answer is yes! Remember management could have sacked or sanction the doctor because the person went against protocol and in the jungle it's always about survival not morality.

You are such an idiot to assert that! What he is saying is true. An accident victim came, you could not even at least do the following:

-Secure IV access, since they may be loosing blood.

-Take blood for urgent PCV, grouping and cross-matching for any emergency transfusion that the critical ones you triaged may need.

-At least examine the patient as a whole, look out for areas of injury, palpate the abdomen and make sure there is no guarding that could suggest internal organ rupture like the spleen.

-Provide pain relief etc etc.

-Did they not teach you the ABC of rescusitation? You don't know that A starts for

A- Airway
B-Breathing
C- Cardiac function
D - Drugs
E - Examination (Quick)

- How stupiid are you that critical patients would be brought to your center (Accident and Emergency), and the first thing you are saying is for them to be referred? Is your work place a Babalawo center that you have to refer them at just sighting them? Won't you make sure that they are in stable condition first.

-You are stupidly calling on the government - to do what? Are they doctors? Do they know how to rescucitate patients? you foolishly called your lazy asss medical officer...what do you expect... Infact, if I am Seun and moderators, I would ban you from Nairaland!

-You are a house officer for goodness sake, the first on call, you ought to have stabilized the patients before calling anyone.

-You should be ashamed to call yourself a doctor. If anything you and the hospital should be sued and made to repeat your housemanship!

Nonsense!

-CramJones
I understand your point of view, but I guess you don't practise in the country presently. I'm not absolving the guy of any wrong doing but I can assure you I've experienced first hand lack of those essential things you mentioned, and yes in an emergency unit, infact so worse there was no needle and syringe. One of the solutions I think to this problem is for we doctors to be proactive ourselves. During my house job then, many of us had side bags we carried about that contained some of these essential things, bought with our money of course, and these were used for patients in emergency situations and when patients get more stable and was able to get his/her own stuffs, we just replaced some of these things we used for them in preparedness for the next patient.
The doc might have just started house job and had not experienced this kind of emergency preparedness before, let's just be easy on him and hope he will pick up some lessons from this thread.

All of u blaming the OP have not been to a govt hospitals to see how things are been run there. that your brother or sister is dying now is not their business, because before they touch the patient you have 2 first obtain a hospital card, then go pay for whatever they will use to treat your love ones. My people it happened to me 2010 when my father was at the peak of dying, none of them(doctors) care for the dying man. So don't blame the young man, he is just receiving orders & if he violates any, he will be fired.

My country don't value life, our government continually killing us, we seem like nothing to them. We can only get better when we do the right time, with the right people. Imagine, how is money more important to life, greed. Actually, it takes just you and I to make the difference, as we wish for a better and great nation. For the lost souls, may they rest in peace, amen. Pax!

There are too many emotional displays on this thread. The OP should not be blamed , he is too young in the profession. However, i believe someone should take up the challenge to sue the senior doctors on call that day...this will be a way to start.

Boss13:

Madam for final advice to you is this - if you are wealthy, do not patronize Nigerian hospitals. For little illnesses, there is a compendium of treatments by well recognized and reknowned world class doctors, you can subscribed to their services online. You can be their patient too. They can assure regular phone call check-ins, video chat or you can travel if the illness is complicated.

If you are in the process of acquiring wealth, first get a health insurance. Second, browse for very good hospitals in the country. When you visit, be inquisitive. Insist on test before treatment and always read and research your test results and compare treatments. Doctors in Nigeria can diagnose wrongly.

Finally, NEVER give birth in Nigeria If you are married and still breeding. If you ain't, put it in serious consideration.

Take care
This Negro has sold out mehn! Never give birth in Nigeria? just ridiculous. Its people like this that are the basis of the depreciating value of our Naira.

As far as Nigerians are concerned every hospital visit is an emergency. Headache that won't go away can take anyone to the ER. Most people only go to the hospital when they are sick(with the exception of annual pap smear for women and other wellness stuff) Once you are sick, it would be too late for doctors appointment that could take days. Nigerians don't really do checkups.
