As much as I don't like passing judgment on God's matter, there is no way it can be proved that there was no compulsion based on the wording of the form.

The least I expect is that the Pastor in Charge be suspended indefinitely and demoted as well. These are the kind of people giving the Church a bad name therefore making evangelism difficult.
