Chelsea emerged Premier League champions after a well-deserved 1-0 win against West Brom at The Hawthorns, West Bromwich.

The win was sealed when Belgian striker Michy Batshuayi came off the bench to score the winning goal that secured another Premier League title for Chelsea.
