Though is called green form but its a white form. ℓ̊  just filled mine. Go  τ̅☺ admission office at senate building i̶̲̥̅̊n̶̲̥̅̊ oau. Go  along with a passport photograph,ur credentials, ur reg slip and probably ur transcript.

Hello,for those usin awaitin just be sure ur result is out before d admission process and neva use awaitin result,just fill in d one u hav as at hand cos is likely u ar disqualified if u fill awaiting result.and pls ur transcripts is equal to ur admission,pls ensure u forward ur transcripts and it get to oau,den lastly is PRAYER.best of luck guys

It is free. for τ̅ђё passport, τ̅ђё background colour wasn't specified. ℓ̊  took my passport i̶̲̥̅̊n̶̲̥̅̊ τ̅ђё school and it was a red background dat was used. U̶̲̥̅̊ will Ђåvε̲̣ τ̅☺ summit ur reg slip o'level,ND result and transcript. U̶̲̥̅̊ can summit ur transcript later but make it fast.

Second, the applied geophysics department of OAU only accepts HND candidates, does this mean I gotta submit both of my HND and ND transcripts?

U̶̲̥̅̊ only summit τ̅ђё original O̶̷̩̥̊͡Ƒ ur transcript. Wen u̶̲̥̅̊ going τ̅☺ oau just go  along with both transripts and ask which τ̅☺ summit.

my advice my for Ɣ☺Ʊ is τ̅☺ go τ̅☺ OAU yourself º°˚°˚˚°• don't send anybody. I also live i̶̲̥̅̊n̶̲̥̅̊ Lagos and I've filled mine

Hello evryone, have filled the D.E form(aka Green form, although its a photocopied white form) @ OAU today nd have submitted my transcript also. For those who are still processing their transcript, try go to fill d form @ d admission offfice before your Transcript get to IFE. Delay might be dangerous! I applied for Elect/Elect 1st choice nd Computer Engr. 2nd Choice. lets all keep informing each other as infos pop up. best of luck...
