Most people would say state of origin, which will definitely include details of your local government area... While alot more will use local government of origin which is more precise and give more details as to your clan.........

Just recieved mine, cross river. 24th is the date for screening, but i don't know if my statment of result(degree) will be accepted. I graduatd since 2012.

A friend called their customer care OK this. Only accepted if not more than 5 years after graduation.
