(1) Certificate of Origin
(2) Sworn Declaration of Age
(3) 2 letters of recommendations 4rm 2 reputable persons to be directed 2 d Registra
(4) Original and Photocopies of ur O'level(WASSCE, GCE, NECO)
(5) Original and Photocopies of ur Jamb coloured result print out
(6) Jamb Admission letter (Original and Duplicate)
(7) Up to 10 recent Passport photographs
( Letter of Personal Undertaking for Provisional Admission
(9) Bank Tellers confirming d payments of 2000 post Jamb form nd 10,000 acceptance fee
(10) Letter of Invitation for Post UTME
According 2 last year, everyone of dese documents are important, as UI will not screen u without dem. Goodluck

tufe: You don't have to go to delta. I'm Deltan too and I did mine some 3months ago here in lagos, at V.I for N1000. But u may sort one stupid guy there about N200 naira. Go with ur birth cert and 2passports. Below is the address. They may also ask you to count 1-10 in ur language.
