Remember this is immigrant visa not nonimmigrant visa.
For immigrant visa the visa validity is for 6months from the date of medicals. YOU HAVE TO LAND IN THE USA BEFORE THAT 6months expires.

Its when you criss the border that you are now permanent resident ftom the date you arrive at US border.

This is not quite correct. Your visa category doesn't change because of the event of childbearing. It only changes when there is a change in martial status.
