I did my biometrics on the 30 September. I received a mail on the 11th that saying

I’m wondering that we were not asked to give passport pictures. It was supposed to be taken during biometrics. What do you think can be responsible for this? Any contributions or advice will be appreciated

2. Do I need to submit offer letter or the CoS is sufficient ?

3. Do I need to submit a cover letter with my visa & immigration form?

4. For dependant below 18 , do i need submit a sort of parent consent letter ?

1. Use your employer's address

2. You don't need to submit CoS. It is optional and submitting it could only help you in case there is any error in your application. Offer letter is of no use.

3. NO

4. Birth Certificate and Passport is enough if both parents are going. If not, the parent that is not going will need to write a consent letter.

They are actually supposed to send you the COS. I don’t see why not. It is not just a series of numbers. It’s an actual document. There you would also get their sponsorship license number, see if they ticked maintenance and all what not. From my experience, it is sent. It was even from the COS we found out that we were getting 5 years contracts with the nhs trust.

Just in case it is one of these scam schemes trending, make sure you get your COS and not just a number because I know people are desperately paying for cos now.

Hmm... Quite an enormous number of requirements! Congratulations to you.
Was it NHS Scotland or an NHS Trust that offers an additional 'high cost area supplementation'? I'm asking because most other NHS Trusts, aside from the ones mentioned, typically offer £20,330 for Band 3 roles, which falls short of the UKVI £20,480 salary threshold requirement for HACW visa.

No ... you have to be consistent. COS has to match visa years

Overstay is usually a problem but for skilled visa, the official document states that over staying will not be counted as an offense and would be over looked. This rule i saw it with my two eyes.. lol

Accommodation, my cousin just stated that she will sort temporary accommodation such as air bnb after the visa

The amount of years you can apply for is dependent on your CoS. They're sponsoring you for 5 years so you apply for 5 years. If after 3 years you don't want to do again, get yourself a different sponsor except if you're miraculously eligible for ILR.

2. That's dependent on your officer. You're to declare all previous visas in the last 10years so it's definitely going to pop up.

3. You'll have to sort out your accommodation yourself. You'll need either an air BnB or an apartment.

I hope you know you cannot use this for skilled worker visa / care visa. This is not an acceptable form to use as proof of English (a mandatory requirement). Mandatory means if you don't have what they are requesting then it 100 percent rejection
