The postal code we are to fill, is it the one for state or local government? Am asking because I used the postal code of the local government I reside. Also in the space for "primary school", are we filling any grades? Am asking because I can't remember collecting any certificate from my primary school. |

Rosarie:I know this question has been answered but in order not to make a mistake that's why am bringing it up again. I went through the form note and the instruction reads "if you have changed your name or used other names in the past, you must enter those names here". Sorry to ask this funny question. In the space for "next of kin", are we to fill more than one entry i.e father and mother? Or do we just fill one, i.e either father or mother? Thanks. Also in the space for "address", after filling that, we were requested to fill in postal code, in my case I filled in the postal code for amuwo odofin which is 102102. Hello Doc, Is there a permanent cure for the treatment of staph. My sister got married about 2yrs ago and during that time, she ran a lab test and staph aureus was seen. She treated it and got pregnant immediately. Fast forward to this year, she ran a test after complaining of mild stomach ache and also did a scan. The urine test and hvs result indicated a yeast infection and scanty growth of staphylococcus seen. She has been placed on drugs that are sensitive to the bacteria. She is confused on what brought about a reoccurence? Please your input would be highly appreciated. Thanks. |

omisakint:am having the same challenge. The day was imputed as 15th instead of 16th. Also my employment history came out in 2 places instead of one. I don't know how to delete it. I have gotten to the final stage of submission but my employment history and institution attended came out twice. I don't know what to do. |
