Good afternoon people. I recently applied for change of name reason because all my documents except for my jamb result carry three names. And as a result I couldnt print out my jamb admission letter which I Will be needing for my nysc. Jamb gave me an indemnity form to fill which I just did. It's stated in the form that I need to take it to my school for my H.O.D to sign. So I just wanna know if it will be okay to swear an affidavit as a proof when going to school.
