CARE COORDINATOR We are seeking a dedicated Care Coordinator to manage enrollees under Life Action Plus HMO. You will be responsible for reaching out to them and determining their health needs, developing care plans, coordinating patient-care services, educating them about their condition, and working with the medical team to evaluate interventions. The successful applicant will display a compassionate nature, be knowledgeable about health care practices, and provide exceptional customer service. RESPONSIBILITIES • Monitor daily admissions and call enrollees on admissions at the end of each day • Follow-up with enrollees on admission and also call them at discharge to ensure they are satisfied with care given at the hospital • Reach-out to specific enrollees to discuss their health problems and develop a care plan to address their personal health care needs. • Monitor enrollees on the pharmacy benefit scheme closely; track their medications from point of dispensing to delivery. • Educate enrollees with chronic disease about their condition, medications, and give them specific care instructions. • Check-in on the patient regularly; evaluate and document their progress of care. • Assist the medical team with developing and accessing health interventions. • Attend ongoing training and courses to keep abreast of new developments in health care. • Handling patient case management and education with the medical team • Maintaining patient confidentiality and quality care • Treat patients with empathy and respect and conduct oneself in a professional manner. • Comply with organizational guidelines and health care laws and regulations REQUIREMENTS • A degree in nursing, Social Work, or related field • License may be required • At least 3 years' experience in care coordination or clinical practices • Good verbal and written communications skills • Computer skills and proficiency in Microsoft Office • Strong analytical thinking and ability to handle multiple tasks concurrently • Excellent customer service • Compassion and empathy • Ability to travel Send CV to recruitment@lifeactionplus.com |

Good Day All, I got a tokunbo Honda CRV 2005 in December 2019, It has been serviced twice and currently due for servicing. I couldn't get the recommended oil, so I use Mobil 1 5w30 on the two occassion, can i still change back to the recommended oil (5w20) or should I continue with the 5w30. Thank you. |
