What awaited May was a “brain gene-editing therapy” that had never been attempted on any patient worldwide. Three days after medical staff injected the treatment into her spinal fluid, she developed a fever. Soon after, she stopped urinating, and her platelet count plummeted. Before being moved to the intensive care unit, May whispered, “Mom, I want to go home.”

May died a week after the treatment, and the incident remained undisclosed for over a year. It was recently revealed in a joint investigative report by the international journal *Science* and Retraction Watch. The tragic outcome of the therapy, which aimed to correct a single faulty DNA base in her gene, has sparked safety concerns.


May suffered from Snyder-Robinson Block-Campo syndrome, a rare genetic disorder caused by a mutation in the CHD3 gene, which delays language and motor development. With only about 200 confirmed cases worldwide, the condition is not life-threatening. May’s symptoms were relatively mild—her drawing and writing skills lagged behind peers, but she could still attend kindergarten. However, her parents feared that as she grew older, the gap with peers might widen, leaving her unable to live independently. After learning about a professor’s research at Shanghai Jiao Tong University through a social media group for families with rare diseases, the parents emailed in 2023 requesting treatment. The professor claimed the gene-editing technology had reached a turning point and that a treatment could be developed. The parents trusted his assurance that a top-tier hospital would prevent safety accidents.


The technology used was “base editing,” which corrects a single incorrect DNA base in the CHD3 gene. The challenge was delivering the editing enzyme and guide RNA to brain cells. Researchers embedded the genetic blueprint into an adeno-associated virus (AAV), a modified virus stripped of its ability to replicate or cause disease.

However, the genetic material required for the therapy was too large to fit into a single AAV. It had to be split into two viruses, both of which needed to enter the same brain cell for the edit to work. This lowered the success rate. To deliver both viruses to enough neurons, the dosage had to be increased dramatically. The team bypassed the blood-brain barrier by injecting trillions of viral particles into May’s spinal fluid through a needle inserted between her vertebrae. She died a week later.


The hospital’s ethics committee concluded after an emergency meeting that May’s death was caused by “thrombotic microangiopathy,” a condition where microclots form in small blood vessels, destroying red blood cells, platelets, and damaging organs like the kidneys. The hospital acknowledged the treatment was directly responsible.

Problems arose from earlier animal tests, which showed warning signs. Before May’s trial, the team tested the treatment on four monkeys. The final toxicity report revealed that all four developed liver damage, and some suffered kidney injury. Experts argue that if such risks were identified, additional tests with lower doses should have been conducted. Steven Gray, a professor at the University of Texas Southwestern Medical Center, told *Science*, “This therapy should never have moved to human trials.”

Yet, the ethics committee approved May’s trial before the final monkey toxicity report was completed. The research director informed the parents about the monkeys’ abnormalities but reportedly downplayed the risks. The consent form mentioned the possibility of thrombotic microangiopathy and organ damage, but May’s parents claimed they were never told the treatment could be fatal. Critics argue that for a first-in-human trial, the risk of death must be explicitly disclosed.


May’s parents were not just trial participants—they were funders. They covered $860,000 (approximately 1.2 billion Korean won) for treatment development, mouse, and monkey experiments. Some payments were made directly to a researcher’s personal account. The parents also gifted iPhones, iPads, and Maotai liquor, and covered meal expenses. Desperate, they pooled money borrowed from relatives to fund the research.

In February of this year, about a year after May’s death, the team published results in *Nature* on correcting the CHD3 gene in mice and delivering the therapy to monkey brains. The initial draft included May’s and her parents’ genetic information, along with a thank-you note for their participation and support. However, the final paper omitted all references to May, her death, and the parents’ contributions. It also excluded the finding that all four monkeys had liver damage.

The parents demanded the paper’s retraction, fearing other families might pursue the same treatment based solely on the study. The research director initially agreed but later stopped responding. After the paper’s publication, Chinese state media hailed it as the “first ray of hope” for families with rare diseases—a claim that prompted May’s parents to go public.

Chinese authorities later fined Xinhua Hospital approximately $3,600 for failing to properly supervise the trial and register it as a commercial study. *Nature* stated it was unaware of May’s death during peer review.

Balancing Risk for Non-Life-Threatening Genetic Diseases

Not all gene therapies are risky. For conditions like spinal muscular atrophy, proven treatments have dramatically improved patients’ lives.

Gene therapy’s high efficacy comes with significant risks. For patients facing imminent death without treatment, even severe side effects may be justified. However, for cases like May’s—where a normal lifespan is possible and symptoms can be managed through rehabilitation and special education—the acceptable risk threshold should be much higher. There are concerns that parents’ hopes for their children and researchers’ expectations might cloud judgment of true risks.

In May’s case, the informed consent process failed to disclose the possibility of death, and research ethics regarding transparency after her death were not upheld. While gene-editing technology advances rapidly, safety safeguards have yet to catch up.
