I have no clue for the first question, but for the mistake its from your school, we never fill anything concerning grades, they appear on its own...have you collected your statement of result ??

But to be on the safe side, just regularise. You never can tell if this will affect your registration. It won't cost you anything except to leave your house and pay little cash. Your details with Jamb should correspond with the one your school is sending to nysc. How can your Jamb admission letter say you went to one school and you're presenting a result from another school?
