Babies are fed on demand ,u don't wake a baby up just to feed her. God created babies in such a way that when they are hungry or need attention ,they will cry .
U talked about diapers ,for u to know when it needs a change;check the weight of an unused diapers then if the one she's putting on becomes heavy that means its time for a change.
I also gathered that the baby is malnourished and if I must be frank with u , a nanny is just one out of the many things u need . Go see a health care provider and most of all a dietitian to help put this baby back on track.good luck and happy baby sitting
