Both, you show them your medical certificate and you then they do little screening, like checking your blood pressure, temperature, height and weight

I did mine yesterday, there was nothing much about it, go as early as possible, the time on my letter was 2pm, I got there around 1:30 they were already rounding up, the officers are friendly
