A lot of women in Nigeria, both old and young, are suffering from this. Therefore, we have deemed it fit to release a write-up that will educate every woman about likely causes of scanty menstruation and spotting and how they can avoid them. Scanty menstruation, otherwise known as Hypomenorrhea, is a condition in which a woman suffers from very scanty discharge during her menstrual periods. The duration of the menstrual period may also become shortened as a consequence of this problem. Women suffering from this problem also commonly experience vaginal spotting during their periods. Hypomenorrhea is the reverse of the more common problem of excessive menstrual discharge faced by many women. In some women, the problem may be hereditary and it may be found upon investigation that other female members of their family, like mother and sister(s) also have a history of hypomenorrhea. It is also possible that the problem lies in the constitution of the endometrial vascular tissues. In a few cases, the problem results from the fact that the area of the surface within the reproductive tract from which bleeding takes place during the menstrual period is itself restricted. This could be the consequence of a previous surgery conducted on the uterus or any other reproductive organ. However, if there is some irregularity in the hormonal activities in the ovaries, chances are that the person would experience irregular periods rather than scanty flow of blood during the periods. Women nearing menopause are more susceptible to hypomennorahea than their younger counterparts. This is because the female reproductive tract begins to produce a thinner lining of cells and blood during the menstrual cycle as the body grows older, resulting in decreased menstrual flow during periods. While there are many causes of infertility, blockage of the fallopian tubes is often the reason why many women are unable to conceive. The fallopian tubes are the pathways in which the ova (eggs) travel from the ovaries down into the uterus.Therefore, if there is a blockage in these tubes, it can prevent fertilization from occurring. The fallopian tubes can sometimes become blocked or even damaged due to certain conditions that a woman may suffer from. In rare cases, the blockage to the fallopian tubes may have been present since birth as a birth defect, but have gone undetected until the woman reached adulthood and tried to conceive. Upon ovulation, the egg travels from the ovary to the fallopian tubes where the sperm will meet the egg and fertilization occurs. Once fertilized, the zygote (fertilized egg) is pushed through the fallopian tubes into the uterus where implantation will occur. How Cells of the Fallopian Tubes are Affected by Hormones There are two types of cells within the fallopian tubes. Ciliated cells and Peg Cells. Ciliated cells are most abundant in the infundibulum and ampullary. Estrogen increases the production of cilia cells in the fallopian tubes. Scattered between the ciliated cells are peg cells which produce tubular fluid. This fluid contains important nutrients for both sperm, eggs, and zygotes (fertilized eggs). The secretions also promote capacitation of the sperm. You may not know it, but the sperm cannot mature for complete fertilization without this important fluid. Progesterone increases the number of peg cells. Estrogen increases the height and secretory activity of the peg cells. In addition, tubal fluid flows against the action of the cilia, near the fimbriated end. Not only is progesterone and estrogen balance vital to the menstrual cycle overall, but it is vital to the health and proper function of the fallopian tubes as well. My goal in writing this article is to help you read your body and listen when it is telling you something is out of balance. The menstrual cycle is one of the best ways the body communicates about your fertility. The details of your menstrual cycle can be used as a window into your fertility. Telling you just what is going on, where an imbalance might lie, how your reproductive circulation is, if there is a hormonal imbalance and so on. Missing A Period Missing one period is no cause for alarm. The hormonal balance is very delicate and can be affected easily by stress. It generally will reset itself. Missing one or a couple of periods does not imply infertility necessarily as you can still ovulate even if you are not menstruating. Missing a period for 6 months or more is called amenorrhea and is covered below. Short Cycles Short cycles can cause fertility issues due to lack of ovulation, too short of follicular phase or luteal phase, lack of nutrition, deficiencies, anemia or low body weight. Uterine fibroids are benign tumors that grow outside, inside or within the wall of the uterus (womb). Composed of muscle and fibrous tissue, these growths are sometimes known as fibromyomas, leiomyomas or uterine myomas. Scientists are still not certain why fibroids occur but there is some evidence to suggest that the over-production of the hormone estrogen plays a role. As a result, most natural treatments for fibroids, including diet therapy, focuses on reducing estrogen to combat fibroid growth. Progesterone is an important female reproductive hormone. It performs various functions in a woman’s body. It is required to regulate menstrual cycle along with estrogen, another important female reproductive system hormone. Progesterone is also required to initiate and maintain pregnancy. CAUSES OF FEMALE INFERTILITY Failure to Ovulate Ovulatory disorders are one of the most common reasons why women are unable to conceive, and account for 30% of women’s infertility. Fortunately, approximately 70% of these cases can be successfully treated. The causes of failed ovulation can be categorized as follows: (1) Hormonal Problems These are the most common causes of anovulation. The process of ovulation depends upon a complex balance of hormones and their interactions to be successful, and any disruption in this process can hinder ovulation. There are three main sources causing this problem: Failure to produce mature eggs In approximately 50% of the cases of anovulation, the ovaries do not produce normal follicles inwhich the eggs can mature. Ovulation is rare if the eggs are immature and the chance of fertilization becomes almost nonexistent. Polycystic ovary syndrome, the most common disorder responsible for this problem, includes symptoms such as amenorrhoea, hirsutism, anovulation and infertility. This syndrome is characterized by a reduced production of FSH, and normal or increased levels of LH, oestrogen and testosterone. The current hypothesis is that the suppression of FSH associated with this condition causes only partial development of ovarian follicles, and follicular cysts can be detected in an ultrasound scan. The affected ovary often becomes surrounded with a smooth white capsule and double its normal size. The increased level of oestrogen raises the risk of breast cancer. Malfunction of the hypothalamus The hypothalamus is the portion of the brain responsible for sending signals to the pituitary gland, which, in turn, sends hormonal stimuli to the ovaries in the form of FSH and LH to initiate egg maturation. If the hypothalamus fails to trigger and control this process, immature eggs will result. This is the cause of ovarian failure in 20% of infertility cases. Malfunction of the pituitary gland The pituitary’s responsibility lies in producing and secreting FSH and LH. The ovaries will be unable to ovulate properly if either too much or too little of these substances is produced. Millions of women suffer from blocked fallopian tubes, and this condition is one of the most common causes of female infertility. Fallopian tubes can be blocked because of scarring caused by injury or infection, but the underlying cause of the condition is sometimes unknown. Although common, blocked fallopian tubes are rarely a cause for serious concern. Because blocked fallopian tubes are often asymptomatic or present only with very mild symptoms, the condition is under-diagnosed, however it is a leading cause of infertility. Infertility Caused by a Blocked Fallopian Tube Women who are not trying to conceive may be completely unaware of their blocked fallopian tubes, since infertility is often the only symptom. Since a blockage in the fallopian tubes can prevent sperm and eggs from traveling freely, they may be unable to join and create a zygote. Women with one occluded fallopian tube may have difficulty conceiving, but if the other tube is fully functional, the patient may never experience noticeable infertility. Unexplained infertility is a term we hear a lot in the reproductive world. Basically, it is the term used when there is no found cause for infertility. Usually, a couple has gone through all of the testing available and everything comes back clear. This is good news, but can be so frustrating if you have not gotten pregnant yet. In the case of unexplained infertility, it can be hard to figure out what to do next. A diagnosis of unexplained infertility is given to about 25% of couples with infertility. Unexplained Infertility: The Basics Sometimes it could be as simple as a nutritional deficiency, low cervical mucous, inaccurate timing of sexual intercourse, a pre-existing health issue, or it may be that you need to explore options for more in-depth testing. It is important to cover all the basics, including additional testing that may need to take place. We are going to cover key areas that need to be addressed, some areas may or may not apply to you, but some may have not been taken into consideration when you were given your diagnosis. Approximately a quarter of all consultations we do involve couples who are given a diagnosis of unexplained infertility, yet they also have other health related conditions, may have poor dietary and lifestyle habits and many even have fertility issues that directly impact reproductive health. If you have been diagnosed with a fertility issue such as endometriosis, uterine fibroids, ovarian cysts, Fallopian Tube Blockage, Hormonal Imbalances, etc., those issues may be what are causing your infertility struggles. It is very important to address those issues individually, prior to trying to conceive. Hormonal Balance If the hormones are not balanced or the menstrual cycle is off, pregnancy can not occur. If pregnancy does occur, the risk of miscarriage increases. This is important for the male partner as well. Men can have hormonal imbalance as well, which may contribute to difficulties in impregnating a woman due to poor sperm health, or other health related issues. Women Menstrual cycle irregularities may be a sign of hormonal imbalance, or other more serious fertility issues. If any of the below hormones are not at the correct levels, the menstrual cycle may become irregular. I want to begin this article by first examining the relationship between Birth Control Treatments and Female Reproductive System, especially those Birth Control Treatments that contain hormones. While I feel that it is important for you to have the freedom to choose whether or not you want to be pregnant during certain times in your live, the more I hear from women who’s menstrual cycles have been disrupted from the use of birth control pills/therapies containing synthetic hormones makes me wonder if there might be a better way. If we could educate the public, delve deeper into understanding what the excess hormones do in your body, as well as learn alternative methods of preventing pregnancy that may be more beneficial to long-term health, we might be able to prepare for conception in a healthier way. Birth Control and Female Fertility Many doctors will easily prescribe oral contraceptive (pill) birth control if you have fertility issues such as PCOS, Endometriosis, Ovarian Cysts or irregular menstrual cycles. While the birth control may work for some time for those conditions, the birth control does not address the key issue of why you have that fertility issue in the first place. It may reduce pain, reduce the incidence of ovarian cyst or regulate your cycle for some time and then when you stop taking it, your symptoms may come back. When I speak of birth control, I am speaking of the kinds that contain hormones or IUD (which may affect the uterus and menstrual cycle). Birth Control Pills, Intrauterine Device also known as IUD (Copper T, Mirena, Progestasert), Implant (Implanon), The Ring (NuvaRing), The Patch (Ortho Evra) and Injectable Progestin (Depo-Provera) all contain synthetic hormones except for the Copper T IUD. Anytime between the time you stop using it and up to a year afterward is considered a normal time frame in which it will take for your body on it’s own to regulate your hormones and start a regular menstrual cycle. If you were taking birth control to help regulate your cycles or for other fertility issues, this average may not apply because your cycles may have not been regular to begin with, and there are no guarantees that the birth control use is going to keep them regulated like when you were using it. For some methods like the Implant or Depo-Provera, some women do not get their menstrual cycle at all and it seems that these women have a harder time getting their menstrual cycles to come back. The growing rates of infertility and secondary infertility (infertility after a woman has had one child) can be attributed to many things, but many medical experts and researchers are reluctant to name surgical abortion as a possible causal factor. Conventional treatment for fibroids often leaves many couples feeling disillusioned as very often, women are given options which are disappointing to say the least. When a woman has damaged fallopian tubes, it increases the risk of ectopic pregnancy, also called tubal pregnancy. An ectopic pregnancy occurs when the fertilized egg is unable to pass to the uterus, so the embryo grows in the fallopian tube instead. This happens in one out of every 40 or 50 pregnancies and is most likely to occur around the eight week of pregnancy. Symptoms of Ectopic Pregnancy The symptoms of an ectopic pregnancy may initially feel like symptoms of a normal pregnancy, but can quickly change. Symptoms include: vaginal bleeding sharp or stabbing abdominal cramps pain on one side of the body that may spread through your belly dizziness, weakness or fainting If you have these symptoms of ectopic pregnancy, see a doctor immediately! The major health risk of ectopic pregnancy is that the fallopian tube will rupture and lead to internal bleeding. You may need emergency surgery to reduce the risk of heavy bleeding (hemorrhaging) if the tube ruptures. Ectopic pregnancy is a leading cause of pregnancy-related death; however survival rates have drastically improved due to advances in early detection. An ectopic pregnancy cannot be saved. If the tube has not burst and the pregnancy hasn’t progressed very far, you may only need laparoscopic surgery to remove the embryo and repair the damage. If, however, the tube has ruptured, it may have to be removed. Pregnancy after Ectopic Pregnancy You can have a normal pregnancy after an ectopic pregnancy. If you still have both fallopian tubes, you have a 60 percent chance of having a successful pregnancy in the future. Even if one fallopian tube was removed, the remaining one should be enough to let you get pregnant again. Your doctor may suggest you wait from three to six months before trying to conceive. Even if you have problems with your fallopian tubes, it is possible to become pregnant. Surgery can help by removing a blockage, reconstructing the damaged end of a tube or removing scar tissue. In vitro fertilization (IVF) is another way of trying to become pregnant. This advanced reproductive technology doesn’t repair the tubes but bypasses them by transferring the embryo directly into the uterus. But around 14 days after your period ends — around the time of ovulation — your vaginal discharge will turn clear and become watery and so elastic that it can form thin strands or ropes. Fallopian tubes are slender tubes that extend from the ovaries of a woman to her uterus, and through which eggs pass. Blocked fallopian tubes will prevent this process from occurring and result in infertility in a woman. There are two tests that are most commonly used to determine if a woman’s fallopian tubes have become blocked. Up to 1/4 of female infertility cases are as a result of blocked fallopian tubes. Tubal blockage, along with tubal scarring, can be brought about by such factors as infections of the pelvic region, scar tissue formed after a pelvic surgical procedure, or a condition known as pelvic endometriosis. This is a disease that sees some of the tissue that lines the uterus spreading to the ovaries or to other parts of a woman’s reproductive system. Statistics show that as many as 40 percent of infertile women have this problem. Hysterosalpingogram (HSG) The hysterosalpingogram (HSG) is normally the first test that will be employed to discover if the fallopian tubes are blocked. It is performed in hospitals using X-rays and a contrast dye. The dye will be injected through a woman’s cervix during a hysterosalpingogram and into her uterine cavity; all the while, she is being monitored via a series of X-rays. Open fallopian tubes will allow this dye to pass through to the abdominal cavity, which the X-rays will allow the technician to see. Even if the hysterosalpingogram shows that the fallopian tubes are open, it does not necessarily mean they are still normal and functioning correctly. The lining inside fallopian tubes can be badly damaged but still allow the contrast dye to make it through. If they are severely scarred, they may not be working properly, and could be unable to transport an egg into the uterus. Scarred fallopian tubes can cause what is called a tubal pregnancy, in which the fertilized egg becomes lodged in the tube and starts to develop–a potentially dangerous situation. Complications And Management Of Fibroids In Pregnant Women In some women, uterine fibroids do not always cause obvious symptoms, meaning they can be asymptomatic at times. Consequently, the first time a pregnant woman may discover she has fibroids is when she goes for her first routine prenatal scan. Fortunately, most fibroids are harmless and cause few complications during pregnancy. Some women diagnosed with fibroids before becoming pregnant may worry that pregnancy will cause the fibroid(s) to grow more rapidly; particularly as excess estrogen hormones are a known risk factor. Recent research indicates that most fibroids (80 percent) do not actually increase in size during pregnancy and those that do, shrink back to their pre-pregnancy size again after childbirth. If a fibroid is going to grow, it is likely to do so in the first trimester of pregnancy. It is estimated that one in every 1,000 pregnant women has fibroids, although this figure may be underestimated. Fibroids are more likely to occur in women experiencing a pregnancy after 35 and those who underwent infertility treatment. It is estimated in both categories that the incidence rate could be as high as between 12 to 25 percent. Common Complications Common fibroid Complications Abdominal Pain Many women with fibroids notice increased abdominal pain when they become pregnant. This is due to a condition technically known as red degeneration of fibroids. It is caused by the hemorrhage of blood into the tumor which starves the surrounding tissues. It affects about 10 percent of all women with fibroids during pregnancy. Most commonly it occurs in the last trimester or when fibroids are over 5cm in size. Symptoms may appear similar to those of a twisted ovarian tumor or acute appendicitis. That is: sudden pain, a general feeling of unwellness and fever. The tongue may be dry and the pulse races. Constipation and tenderness in the area of the abdomen where the fibroid is located are also signs. Although, the pain is unpleasant, it is not usually anything to worry about. Do always report any pain or bleeding to your pregnancy healthcare team. Bed rest for 3 or 4 days in addition to safe pain relievers (analgesics) is usually prescribed. Abdominal Pressure As the baby grows and space is restricted in mother’s body, fibroids may cause an uncomfortable pressure as they squeeze on nearby organs. A sharp pain in the legs and lower back may occur if nerves become restricted and compressed. Type, Size and Location Receiving an accurate fibroids diagnosis is important as this will provide information on the type of fibroid growth, its size, how many there are, and where they are located. One study carried out in 1993 investigated 12,500 pregnant women with fibroids and discovered in the vast majority of cases (88 percent), only one fibroid was present. They reported an increased risk of pain and bleeding during pregnancy in women with large fibroids measuring 200cm cube volumes or more, or when the fibroid was located underneath the placenta. For this reason, knowing as much as possible about the type(s) of fibroid present will help a physician predict how it is likely to react during pregnancy. For example, submucosal fibroids, the least common type, can grow through the lining of the uterus and make the cavity inside the womb too small for the baby to grow in over 9 months. In fact nearly 40 percent of pregnancies with submucosal fibroids end up in miscarriage (often before the woman even knows she is expecting). Subserosal fibroids project out of the uterus wall and can grow quite large. Although they do not affect space inside the womb, they are more likely to result in abdominal pain or pressure. Pedunculated fibroids are attached to the uterus by thin stalks, they do not tend to cause problems in pregnancy although if the stalks twist (fibroid torsion), they can cause a severe sharp pain. Less Common Complications Miscarriage The chance of spontaneous miscarriage is greater in women with fibroids. Research indicates that multiple fibroids increase the chance by nearly 24 percent compared to a single fibroid at 8 percent. The location of the fibroid is important as submucosal fibroids for example can increase the rate of miscarriage significantly. Why fibroids should increase the chance of miscarriage is not clear but it may be because the tumors increase irritation of the uterine or they have a compressive (squeezing) effect on the womb or they may reduce blood supply to the developing placenta and fetus. Fibroids (leiomyomata uteri) are the most common benign tumors of the uterus. They usually come in a variety of shapes and sizes, as well as numbers, in the uterus. Very rarely do they turn to cancer – the exact incidence being less than 1% in a patient’s lifetime. There are some fibroids that are located underneath the surface of the uterus, which are called subserosal; these generally do not cause bleeding but can cause pressure. There are those that are embedded in the muscle of the uterus, which are called intramural. Finally, there are those fibroids that affect the lining of the uterus, which are called submucosal, and these are the ones that usually present with excessive bleeding. Symptoms and Tests The most common symptoms of fibroids are heavy bleeding, pressure, increased frequency in urination, pelvic pain and infertility. Although, most fibroids do not usually cause pain, but if they outgrow their blood supply, it can cause pain due to degeneration (which means tissue breakdown).
