Early years of setbacks and accidents — especially in 2000, when two crashes killed 23 Marines — triggered a major Pentagon review and subsequent design changes for the aircraft that served in Iraq and Afghanistan.

During the Gulf War and the Vietnam War there are recorded incidences of situation where US troops were killed by friendly fire.

No country is immune from such mistakes!

You can’t avert this with software based systems. Bugs in few lines of codes can result in glitches at runtime. These modern jets have huge synthetic backends with complex recurring transactions.., as opposed to analog and mechanical based systems. If Microsoft Windows, as at about 5 years ago, could still have a defect density of over 1.5, then these sophisticated crafts will take a while to get it straight on all fronts.
No amount of debugging, 100% cleans up a software or synthetic frameworks. It becomes glaring when it’s scaled or stretched due to intense or several workloads, running multiple complex processes simultaneously.
They’ll get over it with time.
