You re ignorant to think Americans speak ur above sentences unintentionally... they are very much aware of it, It's just an American forged English just like Nigerian pidgin English. If you notice it's d black Americans that are fond of using those sentences.

Exactly understanding is what matters in language. Someone saying he/she has a slight headache in the leg doesn't necessarily mean he/she doesn't know headache does not apply to the legs.
