My man. I assume you are a patient man. But your patience has to stop at this stage. Your calmness might lead you to an early grave. You need to take drastic actions. Not fighting her o.
I suggest separation for now. If you're the major breadwinner if the house, get a new apartment in a location unknown to her. Where you will stay with your kids for now. Lock up your current apartment when She's not around. Notify landlord and tenants that no one should be allowed into your apartment or try breaking the door. Get your kids and move out. Dont bother about her welfare. After a week arrange a meeting with both family members. Then you attend the meeting with your lawyer to negotiate terms of divorce and your demands if the marriage is to continue. All these while never pick or reply her calls. She will begin to bend. If she doesn't, you better part ways for good.

As experienced married man, I can attest to so many things @op write up but I wouldn't be in a rush to judge hearing from one side alone.
