If you follow the Manga the smile fruit leave it's users permanently deformed even, they remained in a permanent state of their devil fruit(i.e Half beast half man), some even go as far as growing a personality of their devil fruit in their body. Like a talking lion growing on Holdem belly, and that weird looking guy with a sheep as his moustache .
