You are welcome!.....but i dont no how u made such a mistake. First page 1: comfirmation of a valid pin number
Second page 2: you fill in; i. Your full name ii.date of birth iii. O'level result and others
Then third page 3: Is for screening......when, you still hav hope if u no more ppl dont just mention ur problem to them OK? Just Give them ur paper that is all

Yes their is another screening, when you re required to bring all this materials
1. Jamb result/payment of ur acceptance receipt
2. O'level result [if your result is nt complete they can terminate ur admssn at this stage]
3. Leaving cert and testimonial
4. Birth cert.
5. Origin cert.
And other requirement after that you will do blood test and so on......That is what i mean by another screening. Did you understand
