ur name are perfect na...the requirement is to have at least 2 same names...if ur name nd surname are intact nd ur middle name varies,it wouldnt be a serious issue...bt if during clearance u are told to go for affidavit of name or buy correction form.then u can fix all dese issues...about the local govt of origin..any affidavit frm a recognized court would be acceptable....

Documents required 4 uniben clearance:
1.ssce result n a new scratch card 4 confirmation.
2.jamb admission leta
3.jamb result
4.birth cert or age declaration
5.local govt origin cert.
6.afidavit of good conduct.
7.Afidavit of non memba of secret cult.
8.guarantor leta
9.acceptance leta.
10.Enough red backgroud passports(24 copies above of size 1"x1" or a biga size.).

About ur 3names saga.i would advice u to wait till clearance commences.u wud be informed by the officals weither to go for name affidavit or not...
So just be patient nd don't rush so that u don't make a mistake..thats why i avoided going for affidavit now.
