How does one submit back the form? Should it be scanned and mailed back to the same email address?

yeah,d 1st message that u were asked to discard contains d email u ll send d form to...just print,fill,scan and send d form in d 2nd email to d email given to u in d 1st

A reference number that starts with 'Eti' was in the mail I got. Could that probably mean 'Etisalat'? Anyways, if you got the mail, do well by sharing the letter part of your reference number so we can have more info about what we are really into... Though I replied the mail already.
