miriamP: You know, this sums up your contribution in this discussion. When it comes down to it, you're just not very bright at all. Please confirm from your superiors in the nursing profession. Doctors will always confer with very experienced nurses. Nurses will always consult with doctors and will pretty much defer to them in many matters. Your statement above is tantamount to claiming you know more about medicine and the medical profession than most medical doctors in Nigeria. ThIs would be greeted with skepticism even if you were a 30 year veteran of the nursing profession. From a rookie like yourself, that's lunacy. Please stop this show of shame at once. NPs and clinical officers works in teaching hospital along side other medical professionals and those who aspire for independent practice works in remote areas where the laws made it so. They know their limitations the same way other doctors know theirs.....a wound care specialist nurse, NICU nurse, CRNAS and all what nots knows so also the doctors. Anaesthesiologist knows his boundary, gastroentrologist, OB/gyn etc non of them encroachs into other fields of medicine though they are all doctors unless your are specifically trained in that field you're committing quarkery unlike Nigeria scenario where an MBBS graduates will be jack of all trade and master of none. Any case above your scope and designations is refered pronto to the specified specialist. So this is not a case of NP referring cos she can't treat them ....No, it is a case of knowing when to refer to a more knowledgeable associate as most doctors do. Medical model and protocols are not the same. There is medical model of care ...nursing model of care.. Medical follow the disease process while nursing follows the human response to the disease process . Physicians, Physicians assistants, CHEWS and clinical officers follows these medical model during their training and practice. While protocols is a laid down rules as you know which you as a doctor or nurse or any other personnel can follow depending on the hospital policy which can change from one hospital to the other. Protocols in sane climes are not made by a profession alone but each profession will be represented and their opinion sought......merit and the demerit noted before instituting it as the hospital protocols. A lot of protocols abounds eg for emergency, unbooked cases, calling of codes, resuscitation before the team arrives and so much more. Surgeon general is a part of NMA glimpse of the future that will never manifest......It will be countered in such away that the government will even throw in towel to the health sector. 1 Like |

Pls don't join iSsues with the baby nurse she lacks insight that's why I stopped responding to her, However I admit there are some nurses who know their stuff and do their work its those who want to ride higher than they are that feel inferior and tag the dr as being proud cos his job involves passing instuctions to you. Let the real issue be debated not this clowiing by itk's. An ignorant man is dangerous when he isn't aware that he is ignorant. Pls continue clerking and diagnosing and prescribing, we all know how that history goes... Pxt was being managed by her neighbour who is a nurse.... Anybody that ignores the need of health workers working like Siamese twins is tantamount to unfortunate denial. A doctor definitely needs his team. Any miriamP that concocts that nurses do not need doctors is as ignorant as a sterile brain. |

thepathologist:well said bro, i took time out to do some research on us and others and found out that most of there doctors are consultants causing a dearth of doctors in primary healthcare which is why even the doctors themselves are pushing nurses and co to be more independent so they can focus on their specialties once residency training is properly done in nigeria the chews/nurses/whatever can have primary healthcare to themselves |

Check it out too. The definition of PHC was propounded in alma-ata,USSR.. while primary care doctors is equivalent to the general practitioners we have in Nigeria that refer specialize cases to specialist. NPs , PAs are taking up the roles cos doctors don't find it lucrative and satisfying anymore and less doctors are going in for primary care residency. The PC residency is opening up for the NPs already in Florida and in some states too. Am only trying to squash the ideas that nurses seeing and treating patients is unheard of or they are pretending to be doctors. And research also shows that they especially CRNAs and anesthesiologist service are the same. People should search for these NPs cos probably if you travel out since they are many now ...there is every possibility that you will be treated by one of them and they go by Dr so so because they have phd in what they do. 1 Like |

Dis type na 3 credit for Waec dem be dey carry enter School of Nursing (English and Maths no even dey). Almost all of their senior matrons na dese type dem get before dem come change the requirement to 5 credit. The other type wey dey more recent na BSN Nurses. Dis ones been go university small, where Doctors come dey teach dem course like Anatomy, Physiology, and Biochemistry. The thing wey come surprise man for the thing be say na the 3 credit type nurse sabi practical work pass the B.SN ones wey just dey make noise for dis thread, so tay the 3 credit nurse go jus dey shake head for the B.SN ones say na grammar dem sabi blow, but to do the real nursing work, dem no fit. Government wan helep all dem nurses upgrade from RN, das why dem give all dem deadline say make the ones wey no get B.SN try get B.SN, otherwise dem no go recognize dem as nurses again. Das why nurses full National Open University of Nigeria dey look for BSN. |

PT.Awal: No need hating! It's never late to become a medical doctor so that you enjoy the privileges. A nurse after years of practice went back to medical school and she's now a consultant Nephrologist of almost 10years. In any medical team, a medical doctor is always the head by virtue of his/her comprehensive training. This is a bitter pill to swallow especially for those who couldn't make med school or failed out, then became unhappy lab scientist or other health worker. Every health worker is important even the cleaners but that doesn't translate to equal right or ability. In engineering school, a lab technician irrespective of his experience cannot become the Dean inlieu of en:gineer with appropriate qualifications. |

Mrs awesome u seem like a logical person which is rare on nl nowadays, u said doctors no longer find pc residencies lucrative and satisfying which is d truth, most doctors find it boring and kinda routine including nigerian doctors, the problem with d nigerian situation is that d government isnt funding residency training enough or could u pls tell me when last d government built a hospital with capacity to train resident doctors unlike in the us and other developed nations where they are constantly expanding there medical capabilities to meet with the increasing population to the extent that they accomodate foreign trained doctors, so my dear sister or mother comparing america to nigeria in terms of healthcare is like comparing sleep with death. as for NMA, JOHESU and co i really dont care since its no more than a battle of self preservation and i don't plan on practicing in nigeria upon graduation, i only find the banter entertaining |

Before we misinform the public and loose focus in this discuss, lets be clear on some issues (some of which I have addressed in previous quotes) 1. who is the head of the medical team? Correct ans: The Doctor so a senario in which I have a paramedic, an NP, a nurse, a physician assistant and a Physician, who takes charge is the Physician 2. are NPs allowed to prescribe? Correct ans: Yes, but most states in US requires a doctor to co-sign their prescriptions (you understand what that implies). Also they are expected to consult doctors and specialist when necessary. Clients are even advised to ask their doctors if their case can be handled by an NP. In Nigeria, BSC nursing has started and I dont see any doctor losing sleep over it. The truth is because Nurses (RN/RM or NPs) are lower in peaking order than the Doctor in training and competence working within the same structure ( be it pediatrics or geriatrics). 3. the Nigeria M.B.B.S senario. the training of any doctor gives him a certain degree of proficiency in Medicine, surgery, Obs/gyn, Child health, Mental health and Public health. Implies that he should be able to handle a wide variety of cases but the training tells him when to refer. Now we have CME/CPDs. The issues of residency space etc and future structure.............sorry you dont have the privilege to share in those information. 4. why the hatred for the Nigeria Doctor? I bet you cant talk about the American Trained the way you disregard Nigerians. You know your place as a nurse or paramedic in US and even revere the doctor |
