Liver functuin tests (LFTs)
urinalysis
e/u/cr
serum bile salt
serum bilirubin estimation
Abdominopelvic scan

Then we should see a gastroenterologist & an endocrinologist.

Any addition or subtraction? Thanks

bilirubin is part of the liver function tests...

Bile salts cannot be isolated from serum, dont worry about that...

If not that doctors are fighting with the govt, it would've been better to just go to a teaching hospital...

But dont worry, just go to a reputable diagnostic centre, do the tests and scans, then we can go from there...

Also try blending fresh moringa leaves and giving her to drink, she might just be lacking some minerals or nutrients in the body.
She should also eat natural sea or Himalayan salt, it contents lots of minerals to. Some people apply moringa on their bodies for skin diseases, i have never try that one

Medicine is beautiful.
Lagusta what do you think? An endocrinologist should suffice. The cholestasis should be kept in mind though. Labs would confirm.

I took time to read through the comments from others before replying. High blood sugar I.e uncontrolled diabetes can cause itching. However in my opinion carrying out lab tests before seeing a doctor is like putting the cart before the horse. My own humble advice is this; go to a teaching hospital and see a doctor who will refer you to an endocrinologist to fully evaluate her blood sugar control and possible complications. A full blood count and ESR seems to have been omitted from your tests. I wonder why? A condition called polycythemia rubra Vera can cause itching and a full blood count will reveal this easily.
Sometimes making a diagnosis is a long and tortous journey so you have to be patient. In the meantime let her use moisturing soap like Dove and cream without additives such as Vaseline while you await a definite diagnosis. I wish her speedy recovery.

Basically, your result will bring about anemia type of disease, but anyways in don't think PV will be anyway close what is wrong with her as the sign she talked about is just at night, Symptoms of PV may include headache, epistaxis, ischemic or hemorrhagic stroke, angina, myocardial infarction, or claudication (cramping or pain in leg muscles), and none of this is part of what she stated in her mum's case..... I agree with you in terms of CBC but more detailed Labs needs to be accompanied with it.....

i would've ordered for a CBC if need be, from the history...

A CBC may just bring out what we expect... anaemia!!!

So why waste the patient's money??

Don't contradict yourself.

You can never have polycythemia in someone who has pruritus secondary to Uraemia complicated from uncontrolled diabetes. It will almost always be an anaemia. Because of this, a smart physician would not order a CBC to diagnose the condition, but to know the overall status of the patient.

Why ESR? Tests are done with a basis from what was gotten from the history. The average Nigerian patient has little or no money to spend on tests which may end up not being useful. So why waste their money ?

She's seen a doctor, went to hospital, her mom was even admitted and there was no positive response. She decided to come to Nairaland, to enjoy the brainstorming ability of healthcare providers, who obviously are advising her in their seasoned professional perspective- and have advised her to see a specialist.

I guess she's well directed.

How do her eyes look? Are they white or a little yellowish?

Does she take a lot of water or minimal water?

She may need to do a liver and organ tests it could be a sign of something serious let her go a thorough organ check. Its important

She is suffering from allergy, but she should try this medicine called cenocide with piriton tablet every nite. She should be drinking water well.

Diabetic nephropathy , itching is showing probably that it is more advanced as in end stage kidney disease. These complication usually occurs if the patient has either or both hypertension, hyperlipidaemia, atherosclerosis etc in addition to diabetes.

This is my submission though.

Let her:-----

-avoid low humidity, stress and anxiety, dry skin, irritating fabrics, chemicals, and contact allergens.

-use nonsteroidal topical emollient like ASA to relieve the itch through its cooling effect and topical antipruritus containing------caine preparations eg prilocaine cream.

-proper medical evaluation by a specialist is nnecessary.

For over a month, my mum has been itching seriously, mostly @ night and the discomfort prevents her from sleeping throughout the night. During the day, everything is normal but once it is night, stuff like hives start appearing on her skin, which itches until dawn. Then in the morning, the hives disappear & the itching stops until night time when thesame thing repeats itself. We have used drugs prescribed by doctors, yet no improvement.

Please what else can we do?

The first thing is to check the bed for bedbugs,if it's negative then she should do liver and kidney function tests as build up of toxins can cause itching,I hope they will come out ok though.
Then one last thing
Some women also complain of strange symptoms including "internal heat", heart beating fast,crawling sensations and itching during menopause,she could be going through menopause.

How old is your mum? For all there is, this might hopefully be something as common as one of the symptons of MENOPAUSE.
