As a medical student, I think the questions are cheap for a final exam but it is also hard because they are topics you probably have done about 2yrs ago(400l)

Common things occur commonly. As a Dr, you won't be seeing multiple sclerosis everyday or cauda equina in the emergency room. You're more likely to see Mr Wakanda in different forms or similar form in all his glory.
Better you miss a multiple sclerosis case as a junior dr than to miss a ketoacidotic crisis like Mr Wakanda. Nobody wants you to fail as a student looking for bread in Nigerian medical schools, you have passed already until you show them you deserve to fail. Its only when you start looking for butter on top of your bread as a postgraduate that you are designed to fail until you prove yourself. lipsrsealed
Proper intervention/mgt within 10minutes could save Mr Wakanda, till your oga at the top shows up. That's all needed of you, to hold the fort.
Even a question that appears as easy as severe malaria can cause confusion and mass failure when it comes to management

Not all questions must be Clinical questions. How were you expecting medical students exams to be? You thought it would be Filled with medical forms of quadratic equations? grin No! A question can just be write on Malaria and you'd see people fumbling.

What makes the question tough is the method of answering it. For eg, that 1a question doesn't just end with definition, you'd start with introduction, definition of the fever, aetiology, distribution, pathophysiology/pathogenesis, clinical presentations, diagnosis, differentials and maybe treatment. Since the other two questions are related to it, you don't have to explain them in your 1a. If the exam were to be a Pathology exam, you must add morphology, which includes both microscopy and macroscopy.
So do you see why the question is not cheap as you assumed? It would be difficult to remember everything. Just imagine knowing all these information for so many other diseases, it's so voluminous and very slippery. Besides most of the examiners use close marking, where you can't get above a specific score for the question.

This is why people die like flies in Nigeria. Quacks without degrees roaming the land.

Rheumatic fever involves something called the modified Jones criteria and you have to be deep in medical arts to know what they are and how to make a diagnosis because the presentation can be varied, unusual or subtle.
In the second case, DKA involves pinpoint clinical knowledge. In fact, you can even be given the diagnosis upfront and fail in the management beginning with the siting of an IV line. Im quite sure you don't know the difference between a yellow and green canulla much less discuss what the merits and demerits of sliding scale insulin administration are. How much fluid will you give the patient and how will you confirm he is adequately hydrated ? What are the different kinds of insulin and which will be appropriate for this patient ? Why does he have fever and what will you do about it ? etc etc

The questions look superficially simple but have layers and layers of technicality to them.

Though the question is quite straightgorward which to me is good as compared to what we see in other medical colleges.

Number 2 requires a lot of details.
Also, you can pass the written buy still fail the clinical exams (Long case and short case/Osce).

In yankee Nursing school, this Mr wakandas condition was simulated for us to solve.

Took me forever to stabilize the patient. I was just confused, not knowing what buttons to press. Trust me na, naija style. When I tire, I pressed call the doctor button.

Unfortunately, you cannot proceed to the next level of the simulation without stabilizing the patient in 10mins like u said. Lol

1.Wakanda is suffering from diabetic ketoacidosis (The RPG is 450, and the deep sighing breahting).

2.Intravenous normal saline, liberally, plus insulin on an hourly basis.Then lotsa antibiotics, and treat for malaria. Investigations include full sepsis workup, and also look at the kidneys, liver etc...end organ damage.

3.Retinopathy, diabetic foot, kidney damage, nerve pain/loss, poor wound healing.

Just IV normal saline? A market woman can say same thing, what determines the volume of normal saline? What amt are you going to give? Are you expecting any potassium deficit? Should that be corrected too? So where does you ABG/VBG come in?, where does your hourly monitoring come in that you want to keep pushing insulin every hour without monitoring? Have you heard of sliding scale? What happened to giving oxygen if sepsis has even crossed your mind?
You'll be surprised that supportive management is equally as important
If you answer no 2 the way you did there (because it probably carries about 10-15 of the total 20 marks), you will score only 5 or less. tongue

As usual, any time an issue about doctors is raised in the internet, a lot of people will jump out from their shells of frustration to masturbate over it as it seems to give them a special kind of orgasm.Funny enough, the medical scape in Nigeria is filled with people with little knowledge of medical sciences who actually think they have arrived. Funny enough majority of these people wanted to study medicine in the university but couldn't meet up with that ambition by virtue of their not being brilliant or hardworking enough. Some were hardworking/brilliant enough but were'nt patient enough to keep trying till they secure admission into the medical school. Some equally entered the medical school but couldn't bend down to study as it keeps on getting into there head and they were shown the way out. These are the people that end up being frustrated for the rest of their lives, they have so much hatred for doctors and are always like what do they know sef. They re the people tormented daily with inferiority complex. They have chosen not to be happy for life, but, I have good news for such people, it is never too late to study medicine if that is what will make you happy. All you need is to try once more. Experience has shown that no matter how exposed you are in paramedical or premedical sciences, you are not a doctor and there is always a wide gap between what you and a Dr can offer. Those who finally went into medical school even after getting a PhD in premedical courses can testify to this. Its only in the internet that you see a lot of all these E warriors insulting Drs but when you meet them in the hospitals they will be the one bowing down up and down greeting you sir and ma even when your attention is not there and when you dont notice them they will label you proud. We seem to know more than or what the dr knows but when we or our wards fall sick especially after mismanaging them we still rush to the hospital in the odd hours of the night to seek the drs attention, is that not hypocrisy? When you see people that are satisfied with there careers in life they always happy and dnt have time for all these nonsense. The Nigerian drs are one of the best in the whole world even though they are one of the most disrespected, these guys work with very little or nothing and have to just depend on their brain all the time to save lives that is why when they find themselves outside the shores of naija they perform wonders. Honestly I dnt understand while Drs and clinical students alike in this forum should descend so low to arguing with these guys that hv chosen not to be happy, you are only giving them the attention they seek by doing so, cos no matter how you argue it, the uninitiated can never ever understand you, these guys are better ignored......SHALOM......

Any time I see the questions of my seniors in school am always like what kind of simple question is this, until I reach the class and am being given the exam.....then you realise khaki no be leather. That define rheumatic fever there, like someone said, fear no go allow you just define am jump to the next question
