Yes it happened but not semi-final, that was 06/07 season. Liverpool beat Chelsea on penalties in the semi-final and went on to lose to AC Milan in the final.

Those things aren't rare in football. I see mistakes from officials time and time again because they are humans. All these necessities the introduction of VAR. I have seen more awful refereeing decisions in football and it isn't just cool to say that particular ref belongs to the team that gained from such.

If I was Mikel and I know vividly he actually use such racist comment on me but was not found guilty by the FA after investigation, I will still not apologize
