This is doesn't look like a mere dermatology case. He'd likely refer to an endocrinologist. More reason he needs to visit a University Teaching Hospital. If its a case of diabetic foot as suspected, he will need to be seen by the endocrinologist, nephrologist and maybe a cardiologist.
