Your grandad's name is useless here. To fill the form, you need

Your Surname = Last name = Father's name

Your name = First name i.e Bashir

Your other name = Middle name

NB: Don't use your grand Dad's name as your other name

d correct n proper format is d Islamic one, ie Bashir ibn AA ibn AB , meaning Bashir son of AA son of AB, so u are Bashir Fahad Isa.
Any person seeing ds knows that u are d son of Fahad ,who in turn is d son of Isa. That is ur lineage . So as for ur educational forms ur dad's name becomes ur middle name.
NB. but don't create discrepancies between how ur name was written in ur birth cert, primary and secondary cert otherwise u may find ur self in some fix along d way. As for me I realized that only my name and grannd dads names were on my birth n primary cert , so I didn't bother including my dad's name in ssce, jamb, university etc
