I believe it should be ready now, November is a long time, but I haven't had time to go and check. I don't think that tracker matters much at this stage.

Yes.
Data page of guarantor's International passport.
1 passport photograph of guarantor.
Photocopy of National ID or Drivers license or NIN slip of guarantor.

Thanks Artery, Please apart from purchasing and registrating/ processing the DE form. Do UNICAL sale their own forms for DE applicants. This is cause I heard some schools do sell forms just how they do to 100l jambites. Secodly, what my chances of securing admssion to medicine, I had a 4.08 CGPA and 8 distinctions and one credit in my WACE. Please can I av ur number or ur mail adress so as to reach you for more information. Thanks

You're welcome. There's no other form to purchase. JAMB sends the applications to all schools. I really can't say what chances you stand, as I believe there'd be other criteria other than CGPA, but bro, 4.08 is a good CGPA, except they're heartless. Lol. You won't be submitting your transcript at all. During registration they'll request it themselves from your school.
