you may have the check if you activated site authorization on your adsense dashboard. if activated, it prevent ads from displaying on any website you did not authorize

seems like its a new account and you have started sending and receiving so soon. guess you have also added a card to verify it. no panic. it's one of their security measures for new account that is not up to 3 months old. just wait for some days before you try again

there are lots of quiz plugin out there. although I don't use plugin. I use an app for mine. it has html 5 quiz which support mobile.this way I just need to create and upload as a stand alone file to host.
