I think it's because you changed your gig image. Each time you edit your gig it will be removed from search result for period of time, I don't know how long.

for you to see PDF as printer option you need to install adobe acrobat or nitro pdf or similar pdf editor. To bypass that, you can as well save the document as PDF then edit it.

yes, with adobe acrobat pro, you can easily detect the required field.

I had my first account blocked and now using the same app for newer account. I have not had problem since then.
To be on the safe side, just clear the app data and cache.

he needs you to make a fillable PDF. To do this you would need Adobe acrobat pro. You can also add the digital sign feature to the document.
