I initially doubted if he was a doctor too until he mentioned the Ashkenazi Jews. I first learnt that term some years back when I was preparing for my primaries and found it intriguing that a group of people are genetically encoded to be plagued with multiple metabolic disorders. My guess is that he is a doctor without the clinical experience which he will gain with time. Also, I figured from his replies that at some point he realized that he made a mistake but didn't want to give up cos his ego was at stake. You guys didn't make it easy for him as well. I am glad it is over though.

Hi doc. I mentioned trichilemmal cysts because the OP didn't volunteer the consistency of the swellings, and trichilemmal cysts do calcify; whoever sent the OP to Union Diagnostics mentioned "calcified" somewhere in the indication for the x-ray....
But your stuff makes a lot of sense too. I guess it would be an interesting case to see up close. These nairaland people just come here to boast about who is a fellow and who is a medical student, it's tiring.

1. She wants to know how it all started and whether there are other problems apart from that on the scalp
2. How rapidly did it grow

I guess this boils down to clerking you but I really must confess that I don't have the time at my disposal now. Need to prepare the kids for school and rush off to work to see my patients. The med students are also writing their exams today. Perhaps, someone else should take up the clerking.

Can we please end this squabble? In as much as I will like to conribute to this debate, I won't cos the answer is painfully obvious. THERE IS NO LYMPH NODE IN THE SCALP. Someone mentioned trichilemmal cysts otherwise known as wens. I saw that too, but thought twice about it cos the consistency is hard. I am thinking subcutaneous nodules?? Cause. Searched online and got these answers which in truth are no answers at all but close.


LESIONS OF THE SCALP:
Actual neoplasms of the scalp are quite rare in children, but congenital lesions of the scalp are rather common.
Two such lesions require special attention due to the known potential for malignant degeneration. Nevus sebaceous is a congenital lesion primarily of sebaceous glands of the scalp that may transform into basal cell carcinoma. Giant congenital compound melanocytic nevi may occur anywhere in the skin including the scalp. These nevi may give rise to malignant melanomas. Early prophylactic resection of these lesions is recommended to decrease the risk of future malignancy.
In practice, superficial lesions confirmed to the dermis are usually treated by dermatologists and plastic surgeons; it is not until subgaleal or cranial involvement is apparent that neurosurgical consultation is obtained. The following conditions, although not necessarily neoplastic, are frequently encountered by pediatric neurosurgeons.


Scalp Nodules
Subcutaneous nodules of the scalp are common in children. The majority do not possess any intracranial extension or any relation to the neural structures.7 However, lesions of the scalp that on palpation are found to be fixed to the skull, or those overlying the midline may require further neurosurgical evaluation.
Scalp nodules result from numerous processes. In children especially, enlarged lymph nodes may present as scalp nodules, particularly in the postauricular region. There is frequently a history of upper respiratory infection and the lymphadenopathv is self-limited. It is also not uncommon in children to notice very firm nodules fixed to the skull after trauma, with or without skull fracture. These periosteal reactions should also resolve spontaneously.
Multiple painless subcutaneous nodules of the scalp may result from an inflammatory process of unknown origin known generically as "necrobiosis." Necrobiotic granulomas of the scalp are described under man titles including rheumatoid nodules, benign rheumatoid nodules, pseudo-rheumatoid nodules, subcutaneous granuloma annulare, and subcutaneous palisadinggranulomas of the scalp. Histologically, necrosis and degeneration of dermal collagen is the predominant feature, surrounded by histiocvtes, epithelioid cells, and multinucleated cells.

In contrast to true rheumatoid nodules associated with rheumatoid arthritis, these granulomas involve the scalp and pretibial subcutaneous tissue and are not found near the joints. Progression to clinical rheumatoid arthritis does not usually occur. The lesions have a predilection for the occipital and frontal scalp and usually resolve spontaneously. In general, biopsy is not indicated in typically appearing lesions.

Neurofibromas of the scalp may accompany neurofibromatosis type 1 (NFl) and to a lesser extent, neurofibromatosis type 2 (NF2). With an incidence of 1 in 3,000 live births, NFl is a commonly observed condition in pediatric neurosurgical practice. In a patient with NFl or NF2, the discovery of painless, subcutaneous nodules of the scalp does not usually present a diagnostic problem. Occasionally, however, a nodule may be found in the absence of known neurofibromatosis. Characteristic cutaneous markings such café-au-lait spots and auxiliary freckling should be sought although they may not vet be present in young children. Biopsy of a lesion that shows neurofibroma, although not usually essential, may prompt consideration of the diagnosis of neurofibromatosis. Resection of neurofibromas of the scalp may be indicated when lesions are painful (e.g., in proximity to the occipital nerve) or disfiguring. Consideration of biopsy or resection should be made in neurofibromas that have rapidly enlarged because malignant degeneration is possible.

A variety of benign proliferative disorders of mesenchymal tissue occur in infancy and may result in masses involving the scalp. skull, and occasionally, the dura. These lesions are referred to by various terms and have excellent long term prognoses despite initial periods of rapid growth. Infantile myofibromatosis is characterized by diffuse or nodular proliferations of cells resembling fibroblasts and smooth muscle cells. Although considered the most common benign fibrous tumor of infancy, involvement of the scalp and skull is relatively rare. Some lesions appear to arise from the dura and result in significant intracranial mass effect although the brain and arachnoid are not invaded. In these cases, dural resection is recommended because recurrence may occur from involved dural.
Cranial fasciitis of childhood is a rare, benign lesion of unknown etiology that appears histologically similar to the more common nodular fasciitis and infantile myofibromatosis. It occurs predominantly in infancy and may present as a rapidly growing subcutaneous mass, often with intracranial or intraorbital extension. Operation is generally indicated for histological diagnosis and relief of mass effect. Despite an appearance alarmingly suggestive of malignancy, these lesions usually do not recur after removal.

The rash looks like one or more small growths or wart-like bumps (called mollusca) that are usually pink, white, or skin-colored. Now this is getting interesting.
My dear, those lesions can never be molluscum contagiosum. I have spent 6 years as a resident doctor in Paediatrics and I have seen quite a number of molluscum contagiosum cases and they don't look like that. I must say that I am rather surprised it was even considered as a differential diagnosis. Again @op, wait for the dermatologist. If you had come to me, that will be my advice. Those lesions don't look like it is of an infectious origin. An experienced dermatologist can make the Diagnosis just by inspection only. I have sent your kids pics to a dermatologist, expecting her reply.

I really don't want to comment on the exchange between the doctor and the student.... Moving on... The skin lesions do not look inflamed to me otherwise the dad would have complained of pain and redness. If he is willing to wait up to 2 weeks to see a dermatologist, then the lesions are not hampering the boy's usual activities. My guess is that the dad is worried about the cosmesis. Simply put, the scalp lesions are not pretty to look at. I will advise you wait to see the dermatologist. I suspect that the next private clinic you take him to will only bombard him with antibiotics probably against staph. A dermatologist will take a different approach. Let me send the pics to a dermatologist tat I know to ask for her opinion on this. My best guess is that there is a link between those lesions and the hairs cuts that he gets.

For any parent, this subject is usually a very trying and painful one; to have your child raped or sexually abused by anyone.

The two terms are not the same. Rape is the act of forcefully having sexual intercourse with any child below the age of 18 years either orally, through the anus or vaginal but sexual abuse entails much more and also includes

— "the inducement or coercion of a child to engage in any unlawful sexual
activity;
— the exploitative use of a child in prostitution or other unlawful sexual
practices;
— the exploitative use of children in pornographic performance and
materials”.

Just recently, a mother came into our clinic last week Thursday with her 17 year old daughter who had been raped by her boss 4 days earlier. The mother just got to know about it when her daughter opened up and had gone to the police to report the matter. She was subsequently referred to the hospital to get a medical report and to do a HIV test.

The consultant in my unit decided to review this case herself. During the interview, the mom was asked to leave so that we could talk to the victim privately. We learnt from the girl that her father had died some years back and due to financial constraints, her mom got her a job to work as an apprentice to a man who runs a small printing press.

On that fateful day the rape occurred, the man had sent her on an errand asking her to report back to him at his place of residence. On arriving at his place, he made some sexual advances which the girl refused and then he proceeded to rape her repeatedly within a space of 3 hours. Not through with this sordid act, he took some nude pictures and threatened to show it to her family and friends if she dares tell anyone. It didn't end there, as the man kept calling her afterwards demanding for sex or else he will release the nude pictures. The patient had no choice but to tell her mom who immediately went to the police to report the matter.

We calmed both mother and child. We further examined the victim's private area for any tell tale signs of abuse like cuts and bruises but we saw none which was not unusual. We requested for HIV test which turned out negative and other tests to rule out other sexually transmitted diseases and pregnancy. Furthermore, we issued the medical report as requested by the police, counselled them and gave them some drugs.

This was just a brief summary of what transpired.

Cheap tactics. I am disappointed. Ordinarily, asking to return the money is an admission of guilt but in this case has been nullified by his lawyer's stand that they are unaware of the source of money. So, in a way, they are simply returning a lost item, the item in case N400,000,000 naira!!!!

I also agree with the person who noted that that money would have reduced in value. He should also pay for the time and the state's money spent addressing his matter. Simply put, arrogance does not pay.

Again, I know that there is no way he has spinal cord injury. He majestically jumped out of a van the other time for pete's sake. I understand the need to claim a disease we have little expertise on... Neurology cases can be tricky... but why spinal cord injury. I would have gone for a metabolic disease like Adrenal leukodystrophy or played the mental disease card. Rookies

@nita2013 and mzfunshy... The name of the iucd is the copper type. Don't actually know the company name for it. But because of my med history: Hypertension runs in our family and I got a breast lump removed during my school days plus multiple stories of weight gain from women who used the hormonal type, I knew that hormonal contraceptives will be a no go area for me.

For the iucd, copper type (there is also hormonal), the only side effect I had the first time I inserted it was vaginal bleeding for at least 2 months or so. Called my colleague and he laughed at me and told me it was nothing as some women bleed for close to 4 months, also considered normal. It was frightening I must admit but the fear of having yet another baby was much more effective. Moreover, I had no other option. Hubby might not want to try condoms and personally I consider condoms messy and disgusting. I waited it out and it stopped right at about the 2nd month.

Doing mind me and my life stories. Bottom line, just choose what works for your body and medical history.

If they are exactly 3 months, then there may be a need to increase their food intake because their weight is barely optimal and if nothing is done about it, they can become undernourished which we don't want.

Calculated their daily calorie needs at 150kcal/kg/day, and got about 120ml of breastmilk or infant Formula which should be given 3hrly in a day which amounts to 8 times in a day (this is just their minimum intake, it can be more). If you are breastfeeding, perhaps you consider expressing some milk. If you want to augment with infant Formula, fair enough.

Please never make the mistake of giving pap mixed with all kind of things. You will only worsen the issue. Don't want to promote my blog or anything but I am feeling too lazy and sick to paste links. Just right there on the main page, you will see why babies under 6 months should be given only milk and nothing else.

Also, are you stressed out? I can only imagine what you are going through taking care of twins. I have 3 kids and I swear my BP is rising. Just to let you know that stress can reduce the amount of breastmilk your body produces. Also, ur babies can feed off your stress. Just relax. Increase their milk feeds and frequency. They will be fine.

So sorry about this. All I could see when reading this was the red flag you mentioned wc was Extreme difficulty in breathing. Please, rush the baby to the hospital. Suspecting bronchiolitis which happens in infants. Tis the season for it too. Please

hehehe, kinda reminds me of how I got pregnant for my second child even without seeing my period. Play it safe, get one of the contraceptives. Personally, I wait for about a month for my body to settle after child both and then I visit my doctor to insert an iucd. Did it after my second child. Removed it 3 years later and got pregnant in a month. Gave birth to the third one and reinserted. It is still in me for almost 4 years. Planning to remove it soon since we are planning for another baby.

Personally I avoid anything that comes with hormones. Complications are just too messy ranging from increase in weight, hypertension to cancers. So, I always go for the non-hormonal type of Iucd. It comes with its own issues but at least your chemistry is safe. I am currently using one now for almost 4 years.

Most of those our "teething signs" are actually due to infections they pick up by putting dirty things in their mouths. I learnt from my first son and thus with my second, i was very careful of such. He had no symptoms except excessive saliva. We just woke up one morning and saw two teeth up. No diarrhea, no fever. He didn't get the flu till he started school.

My Ekene bobo has some boil like rash on his neck and it seems to be spreading to his back and shoulder some one said its ela and that I should use baby tribotan and also get red hibiscus to bath him with. I am so worried and confused He has been scratching his neck for a while now only for this tiny boils to break out on Monday and as at today it has doubled in number! Looks like furunculosis or bullous impetigo. Get altergo cream and apply twice daily till it leaves. If it is too much, you can add antibiotics to it too. I usually prefer ampiclox. You never mentioned the age so don't know d dose to give. And please never burst those lesions abegi. They spread fast.
Forgot to add that you should make sure no part of the skin is swelling up, hot and painful.

Starting from 4 months is quite okay, although 6 months is the ideal. Why don't you check out these two links to help explain better the different feeding Skills a baby acquires as it grows older plus how exactly you should feed them to avoid any issues.

Hello mamas. Ayam a ftm mum. My daughter is oneweek3days. I noticed some kinda rash on her forehead like heatrash. No it has gotten to her neck n back . M getting scared. I showed the dr wen we went for check. She said it will clear, its common but m jus scared.
It doesn't disturb her and she isn't uncomfortable.

If Your baby is fine all through and through with no yellow eyes, then there is no need to worry. 3 months is the time the PCV is at its lowest in every baby. It will pick up again. Just continue Feeding him like you are doing and the PCV will pick up again.
