what is hypertension in pregnancy?
Someone is said to have hypertension when his/her blood pressure[BP] remain elevated above the normal value when it is measured on two or more occasions 4-6hours apart.
hypertension in pregnancy also called pregnancy induced Hypertension(PIH) or preeclampsia or gestational hypertension is a common cause of maternal morbidity & mortality if not diagnosed early & managed appropriately.
PIH can occur at any point in pregnancy commonly after the 20th weeks of pregnancy but not before the 20th weeks. hypertension in pregnancy before the 20th weeks means that the person was already hypertensive before the pregnancy(pregestational hypertension).

Who are those at risk of PIH
1. it is common in first pregnancy
2. common with advance maternal age
3. Women who change their partner are at increased risk
4. obese women
5. previous history of PIH


what are the symptoms of PIH
Persons with PIH may complain of
1. edema of the leg which can progress to other part of the body
2. headache
3. blurred vision
4. vomiting
5. abdominal pain.


what are the complications of PIH
There are a lot of complications of PIH as it affect almost all the system...
women with PIH may have:
1. visual problems
2. liver disease as PIH can cause rupture of the liver capsule
3. convulsions.. the most feared complication.

How to prevent PIH
Every pregnant women are encouraged to register for ante natal care(ANC) & be regular with their visits.
Good ANC visit will help the doctor detect the condition early & determine how to manage them as PIH is a high risk pregnancy.

Conclusion
PIH is a common condition in pregnancy. It increases the risk of maternal & perinatal morbidity & mortality, With proper ANC, the complications can be prevented.

Any doctor that writes S epidermidis for an apparently healthy patient who can produce mid stream urine with ease (not debilitated nor immunosuppressed) and sample wasn't suprapubic or catheterised as a cause of UTI I will trash and keep trashing such results. If you don't know conditions where S epidermidis can use UTI please do yourself a favour and read up.

By CBC/FBC I know you understand it involves basically two obvious parameter to draw a line with leukaemia; the WBC and the film reading. Sorry woman came back yesterday and said she has had a third party opinion and she was told she had Leukemoid reaction and she's now okay from the nonexistencial leukaemia she was diagnosed with.
I don't claim a master of knowledge, I heed to corrections where necessary and correct people where necessary (it doesn't mean I know more than they do).

I won't condescend into name callings if you know you know and if you don't you use noise to mask your ignorance (defense mechanism).

In the clinic last week a child came with herpetic blister in the mouth and when I saw it I just called the Dr and told her this is herpetic blister and not the common one we attribute to "malaria", sometimes I explain what the hepatitis b panel test result mean to physicians who are only exposed to only knowing HBsAg (it doesn't mean I know more than they do but that's the beauty about learn as it can come even from a "miserable" person).

The truth about being a doctor is that it is not just about passing exams and that is the problem in Nigeria . If you can get the grades to study medicine you are admitted whether you are a psychopath or paedophile. The fundamental trait of carin about people in a humane way is never interrogated and so you find people who maybe should have been vet doctors or Engineers as doctors in Nigeria. The Nigerian doctor we see here on Nairaland is invariably without the temperament one expects in a doctor when it comes to human interaction.

Is it any wonder then that in the US,UK and elsewhere. The Nigerian doctor is brought before disciplinary hearings many more times than their peers.

At the core here is this idea that I scored highly in JAMB, studied and passed man exams and so I am a deputy to God.

The issues are

Hepatitis B is a serious condition that is transmitted byy contact with body fluid

In Nigeria and Africa generally this is very common from mother to child in the perinatal period.

The vrus is very infectious and can be transmitted among members of the same household.

Also from blood transfusion or sharing needles, razors etc. Theoretically it can be got from the barber, and from tattoo or body piercing equipment

The virus causes an illness after which it is either eliminated from the body OR in some cases the individual becomes a "CARRIER" and the virus "hides" in the liver indefiinitely.

It can later cause serious disease of the liver and potentially be transmitted to sexual partners etc.

Infection can be prevented by avoiding risk factors and also by a vaccine.

The vaccine is relatively new and not routinely administered around the world. However some countries have adopted the vaccine as a routine for all children and it is compulsory for health workers like surgeons.


All of this is stuff that can easily be understood by any person with secondary school education and there is no justification for the self indulgent arrogance and insolence we have seen on this thread.


My take is it is a good idea to get the vaccine if available where you are thogh many people find that they are immune having had an infection without being aware perhaps mistaking the hepatitis illness when it occurred for something else e,g malaria.
I do not see any reason why this information should be considered the preserve of doctors.

Much of the science that went into understanding the condition was established by scientists who are not doctors.

I will point out something controversial but factual. Almost 100% of doctors are not scientists. They are technicians who operrate or run algorithms, protocols or guidelines of care that are developed by scientists but they are not scientists.

In facts the vast majority of doctors do not do any single experiment after leaving medical school. What does this tell us?
