Reading through comments here i realized antibiotic abuse is way beyond what is written in books. A time is coming(the time is already here) when no matter the class of antibiotics used, significant progress can't be made with the existing antibiotics.
We need to enlighten people on what antibiotics are & why they shouldn't be abused.
We also have to ban sales of antibiotics in parent(chemistry) medical stores.
Pharmacists should be retrained on proper dispensing of antibiotics(with prescriptions only).

Meaning you do not understand me. I asked if the man was brought into the hospital unconscious. If not, then the doctor may have offered him transfusion during clinic visit. Also, I stated that if the anemia was noticed after the procedure, it then mean that the man started bleeding after the surgery in which case they will still have to offer him transfusion again but since the man can't give consent on account of his altered consciousness, the wife can stand in to give consent. Consent was declined by the wife in dis case. She's not to blame

Ilaje1: A doctor is meant to save life. Some doctors would have gone ahead, you can sue them later. There's a case like that in USA. The doctor's argument is that it's against her ethics not to save a life.
You don't carry out any procedure on a patient without consent. That patient wasn't taken to the hospital unconscious. He was alive & consented to the surgery. He may have been offered transfusion to which he decline. His family has no case here. The wife is innocent too

I listed the below to widen our perspective with regard to case in point.
Chief, the doctor is not to blame. The husband was not brought into the hospital unconscious or was it an emergency? He may have been attending clinics. Transfusion may have been discussed with him during clinic visits which he probably may have declined because of his belief. Consent was taken before the procedure. Procedure was successful. He probably had post operative haemorrhage(most likely reactive) & if that is true, he will still be offered transfusion but in this case, if he is not conscious enough to give consent, his wife will.
The wife is not responsible for the dead either. The husband is responsible for his own dead.
A senator asking for another person to be assassinated instead of seeking for a lasting solution to the problem speaks volume of the leaders we have nowadays.

If the twins are sharing the same placenta and sac they are likely to come between 34-36 weeks, if they have separate placenta and sac its between 36-37 weeks, twins do not get to 40 weeks.
As for me i am going for elective, my doctor mentioned it as early as at 12 weeks and i gladly accepted. Reason is that one of the babies is breech while the other is cephalic, it could lead to complications during labour.
Breech at term & or in labor is an indication(absolute) for c/s. Not a breech remote from term. Your baby can turn before term. So in that case, if there's no contraindication for vaginal delivery why shouldn't you be allowed to labor?

Let her get a referral now, better than if things get complicated. To your question, there are certain factors that determine whether she delivers via vaginal delivery or not 1) Is this her first delivery? It will most likely be a CS 2) Is the first twin in cephalic presentation? 3) Is her pelvis adequate? 4)Medical history such as hypertension, diabetes etc All in all I wish her safe delivery. Peace
Why are you discrediting private hospitals sir? It's very wrong to do that. The consultation who run these hospitals are the same people who u feel are the best when u meet them in government facilities.
It's good to advise the young man to go to a teaching hospital for delivery cos they have pediatric & blood bank services which may not be provided in the said private hospital but to say you have zero confidence in private hospitals, it's demeaning to say the least.
Even in the so called saner climes everyone wants to go to, you don't refer normal pregnancies to secondary or tertiary centers. They are usually taken care of at primary care centers.
The fact that our teaching hospitals provide primary care in addition to secondary & tertiary doesn't make it a standard.
A patient having malaria shouldn't be seen in a teaching hospital.

I can see you don't trust the doctor. How is a doctor who knows the clinical details of your wife wrong when he advised you come with her to the hospital? Or were you expecting him to tell you how to treat your wife's diarrhea at home?

Pls don't misquote me with the term "abnormal pregnancy". It's just that multiple pregnancy is not a normal pregnancy & needs proper monitoring.
sreamsense: What you are saying here is "rubbish" like your moniker. You can not be withdrawn from medicine and go directly to engineering, your subject combination defer. You can be dropped from medicine to courses like biochemistry, microbiology, cell biology etc while you can be dropped from engineering to applied maths, physics, statistics etc.
I had (not too close) friend that was helped due to financial challenges from 300 level medicine to 200 lower class in engineering ( ...due to long leg), but got lots of carry over that made her regret coming to engineering. Throughout the whole world, mathematical related courses/subjects are found more difficult by many. Many students studying law, humanity and even social sciences usually find it difficult to cope with. Maths discouraged many students at junior secondary schools from going to science class.
Many people that choose to study medicine (not all) instead of engineering are conditioned to do so because of lack of in-depth knowledge in maths/ further maths. What distiguishes aspiring engineering and medicine students from secondary School level at Jamb level is just Biology and maths/further maths because they do other subjects together. Which one is more difficult to pass? Maths of course. Even NECO/WAEC and JAMB yearly statistics will tell you which subject students usually find more difficult to pass. No argument! Maths/further maths.
If you ask many who go for medicine/nursing/pharmacy, they will tell you ( with exception of few) they are not good at maths not to talk of further maths or attempting maths at jamb level. Many students that qualify for engineering at University level can easily pass biology and economics than if the table turns to medicine.
I am not ruling out the fact that average students can be found both in medicine and engineering due to influence of parents or admission quota system, but many students above average are usually found in both medicine and engineering departments. Also, nature of engineering practice is even difficult that majority of girls in secondary schools run away from it. Only rugged females usually considered going for engineering, other brilliant ones would have hidden under canopy of economic/biology combination to escape to medicine.
Women are many in medicine than in engineering because women see engineering as difficult and stressful which is true even at University level. It takes government financial commitment to motivate students and women to go into engineering they considered difficult. In reality, considering academic rigour, real-life practice and even in research/invention, engineering is harder than medicine.
Prestige given to medicine is because it deals with human being while engineering deals with engine. Human life has no replacement once handles with carelessness but machine/engine can be replaced even though engineering mistake can be vital and may ruin entire nation or city.

The heart apparently failed according to the report. Heart failure cells ( hemosiderin laden macrophages) and hepatosplenomegally proved that.
The gastric finding was also instructive . That looks like a stress ulcer ( which could be from shock vis a viz MODS).
Also, the parents should be asked what was given to that child . With the desquamation of his lips , could it be some sort of SJS ?? With the same offending agent ( usually a medication) triggering a widespread immunological response culminating in MODS .
This is just my humble submission based on my little medical knowledge. I'd like my chiefs here to say what they think about the findings.
That said, this doesn't absolve the alleged perpetrators of the heinous crime that led to the untimely demise of that young chap of any crime. Justice must be served . This would serve as a deterrent and also help Sylvesta's family to achieve the needed closure.

This is definitely Nephrotic syndrome. He needs to visit a teaching hospital or any other specialist hospital. If treatment is initated early enough, he will go into remission within a month.
