Good morning doc
I currently have a -1.25 and -1.5 prescription for my left and right eyes respectively to aid vision for visual acuity.
But lately I noticed when I wake up in the morning my vision is a bit blurry for hours until around 12pm, I also noticed I see halos and rainbows around light(bulb) just about any source of light.
Please advise on how to proceed
I've been trying to see an ophthalmologist at LUTH but it's taking forever.

This is a different case entirely, I made this inquiry at the Alausa office last week

They only have authority to modify for married,widowed or divorced women
If you are doing for reasons other than the stated above, you would have to go to Abuja.

You can have it done at any NIMC accredited centre
You can go to their website to know the accredited centres close to you

Maybe our issue wasn't similar
Mine was I carried a middle name on my NIN and not on my old passport

I was told I have 2 options
1. Modify NIN to be without middlename
2. Goto Headquarter to add middlename to my passport

My thought exactly was, why couldn't they just add a middlename for me here at Alausa

But you are saying you were able to do name modification at Ibadan, I'm surprised

What was it like please ?

Can I go for capturing at the Alausa Office, before I receive the message ?

Good news. My Wife's passport renewal and change of name is finally out. She captured in Ibadan on August 26.

Finally, the wait of 4 months is over.

Thank you for your response, I think I'm going to stick to that and so an affidavit later, how did you go about the change of name, I was at NIMC Alausa yesterday, those guys are a bunch of unorganized fools.

We stood for several hours at the enquiries section, and no one attended to us

I had a similar issue. Passport had full long name such as Olayemi Chukwuemeka, But my NIN was pulled via the BVN info and it had Yemi Chukwu. Most other docs have the long name, except my bank info. I had to change the NIN info to match.

Please, someone should provide me counsel

I just came back from the passport office at Alausa now

I was told I have issues with my NIN
On my old passport, there is no middle name
But on my NIN there's a Middlename

I have been told I can not proceed like that, that info on both documents need to tally for me to be able to make a renewal

I've been suggested to either apply for correction of data from the NIMC office to remove the middlename or go to Passport office Abuja to add the middlename to my passport.

I'm at a crossroad here
Going to Abuja is a lot of stress and cost, I am in school and lectures are on at the moment

Secondly, I have a middle name on all my other documents and certificates and bank account , removing it from my modes of ID being the NIN and Passport, I hope it won't affect me in future applications ?
